How often do providers miscode preventive care claims? More than you’d think! Preventive services, like wellness visits for adults and children, including labs, should be covered at 100%. But if the provider doesn’t code the visit correctly, insurance may treat it as diagnostic instead.

What does that mean for employees? Instead of full coverage, they could be hit with unexpected costs - deductibles, coinsurance, or out-of-pocket charges, just like a regular visit. It’s easy to blame the insurance company, but in many cases, the issue starts with the provider’s coding.
