“Promise me you won’t let me die,” she said softly. Lisa, a kind and spirited 61 year-old, is the heart of our clinic. She always brightens the room with her humor, her smile so infectious that even on my hardest days, it pulls me out of my fatigue. Last week, she brought me a pair of colorful socks, laughing about how dull my constant black socks were. Despite working as a minimum-wage waitress at a local pizza restaurant, she never misses taking her 9-year-old son to the movies every other week.

Lisa has always been resilient, enduring pain with an incredible strength. But a week ago, she came to me with severe abdominal tenderness, and I ordered a CT scan. When I called her back to discuss the results, she didn’t wait for my words. “Dr. A, I have cancer, don’t I?” she asked, reading my face.

The scan revealed pancreatic cancer, advanced and spreading. We both understood this would likely be our final meeting. Sometimes, life leaves you speechless in the face of its cruelty.
