Blog Artificial Birth Control: Mechanisms and Side Effects ABC Part 2:

So one of the other things I wanted to go over for folks in this session is understanding of how hormonal birth control works and side effects (not that side effects in themselves are reasons against ABC, really Natural Law is, but I do find it interesting the clamor over J&J vaccine blood clots by CDC yet somehow the greater risk for them from many HBC is ignored...). First off, my Youtube Meeting on this topic is here and part of it goes into the sad research that took advantaged on mentally ill or minority women by the makers of the first birth control pill. This research would not be permitted today and many would be appalled at it, yet on the 50th anniversary, there was hardly a mention of this...

First Mechanisms: Although primary mechanisms are not abortifacient, all of them can potentially prevent implantation and although ACOG calls this not abortifacient (because they changed pregnancy definition in the 60s to permit birth control pills), morally speaking human life is human life, not potential human life.
Combined Estrogen/Progesterone (Pills, Nuva Ring, Patches): Main mechanism is to suppress ovulation, but also the progesterone is designed to thicken mucous and keep endometrium thin. Given 8% breakthrough ovulation occurring, effectiveness likely in part could be from preventing implantation (Contraception 1980;22(3):241-7, showed women with breakthrough ovulation endometrium was not receptive to implantation).
Progesterone only pill: Prevent ovulation (though 4/10 still ovulate), thicken cervical muccous, thin endometrium so again can prevent implantation. This is similar with Nexplanon and Depo.
IUD: Primarily acts to make inflammatory response in uterus to cause destruction of s***m, can cause abnormal ova, and progesterone secreting (used to prevent abnormal bleeding seen with Copper IUD), 45-75% still ovulate. Data shows strong likelihood of preventing implantation and those who do become pregnant 50% end with miscarriage even with early removal. It is also used as emergency birth control up to 5 days later so in that setting preventing implantation is likely reason why it is effective 99% of the time. Side effects:
Combined E/P: 3-6 fold increased risk of venous blood clots as well as heart attacks and strokes, Breast Cancer (label as WHO Class 1 carcinogen): •Taking OCP (for 6 years) prior to first full pregnancy found RR 2.2 (first trimester abortion 2.4 RR) by age 32 (Br J Cancer. 1981 Jan; 43(1): 72–76), ever use 7% increase, current use 24% increase
•Use of OCP prior to age 20 increased risk of BC prior to age 50 (HR = 3.26; 95% CI = 1.06–10.01) Breast Cancer Res Treat. 2015 Jan;149(2):505-15
•Although reductions of Ovarian Cancer and Uterine Cancer are touted, overall risk of cancers increase 22% with 8 years use or more (BMJ. 2008 Jan 12; 336(7635): 59–60), Mental Health: •2018 Swedish Study in 815,662 women 12-30 followed for atleast 4 years without prior psychological disorder found 34% higher risk for new prescription for MH disorder with highest in younger girls (3.5 fold higher than nonusers) while very low in adult women. Su***de: 2018 Study (Skovlund, et al. AJP 2017): Follow-up 8.3 years for young women (mean age 21), compared to nonusers of HBC relative risk among current and recent users was 1.97 (97% increase risk) (95% CI=1.85–2.10) for su***de attempt and 3.08 (3 fold higher) (95% CI=1.34–7.08) for su***de. Also some data in young girls in adolescents on the pill it might actually lower Bone Density increasing osteoporosis risk later on. Other Side Effects: Increase in AI disorders (MS 35%, Crohn’s 3 fold increase,50% Lupus), weight gain, decreased libido, GI side effects, irregular bleeding, may affect fertility when coming off.

Progesterone Only: Depo Provera increases VTE 2.5 fold, oral and IUD secreting less data suggesting link. Breast Cancer risk higher with Levonorgestrel use associated with increased breast cancer (93% oral and 21% IUD device) (N Engl J Med 2017; 377:2228-2239). Mood Disorders: •Current use of Progesterone only BC increased risk of mood disorders (OR 3.0 95%CI: 1.1–7.8, p = 0.03) (J Affect Disord. 2012 Sep;140(1):92-6), •Su***de Attempts on oral progestin-only products, 2.58 (95% CI=2.06–3.22), implanted progestin OR 4.42 (95% CI=3.63–5.39), and Depo 6.52 (95% CI=5.03–8.46) (2018 Skovlund).
Depo provera has been linked to low bone density as well, •Other Side effects: Abnormal uterine bleeding, acne, breast tenderness, headaches, weight gain.

IUD: Copper: • Copper IUD early in first 3 months 38% more painful periods and increased bleeding 68%, with 33% reporting more pain and 49% more bleeding still at 40 weeks use,
Both types of IUD: Abdominal and back pain 60-70% with insertion long term 10-15%. •Pelvic Inflammatory Disease: 0.54% PID within first 90 days, 6 fold higher within first 3 weeks of insertion. • Uterine Perforation: 2 per 1000 insertions, slight increase with breast feeding, within 6 months delivery, fewer pregnancies, more abortions
•Expulsion: 1 out of 20 will be expelled within 3 months
•Other side effects: Increased risk of Yeast Infections (19%), Acne 7-15%, breast tenderness, mood changes, nausea, migraines, weight gain, dyspareunia and lower libido
Progesterone secreting: •Breast cancer 21% increase risk of Breast Cancer with Progesterone IUD (N Engl J Med 2017; 377:2228-2239)
Increased risk of depression, anxiety, panic attacks and su***de; (Skovlund 2018) Risk of su***de attempt increased with LNG-IUD OR 2.86 (95% CI=2.06–3.97).

I think the biggest concern is especially in young women. It is amazing how this can have profound mental health side effects and this traditionally has been overlooked, though they come on rather quickly within a couple months. Now granted for instance the risk of blood clots you need to treat 1428 women with OCP to cause one extra blood clot vs in 55-64 yo women on hormone replacement therapy only 655, so we cannot just say well side effects are the only reason not to, but overall when we think about the overall risks (including that small risk of preventing implantation), the alternative in NFP available and the fact NFP used in the setting of a committed relationship helps to nourish authentic relationships while promoting the natural bodily function and generosity NFP use indeed is transforming families in a beautiful and profound way.

Summer is here and slight return to normalcy on the horizon. With Memorial Day around the corner, it is time to think about how we can take advantage of the summer to get closer to that someone special now that the warmth is here and later days to take advantage of:

1. Outdoor Movie: Maybe see if someone has a projector you can borrow and play an outdoor movie for the family or just the two of you. Great to mix with popcorn, BBQ, smores. The days of drive movies maybe in general gone (though they did have a comeback with COVID), but with your own concession stand and chance to snuggle it can make for a warm and unique environment:

2. Pool Time: Want to turn your backyard into a little beach, just get some lawn chairs a little pool for the kids or bigger one for the family inflatable and turn on some nice music, with fun drinks. 3. Fire Pit: Getting a fire pit and some nice lantern or outdoor hanging lights can spruce up the back yard for fun smores, or relaxing evening with wine, cheese and star gazing.

4. Outdoor Play or Festival: Feeling adventurous enough to be with others again, maybe a Shakespeare in the Park, Renaissance Fair, Concert or Rodeo with tasty treats to enjoy.

5. Your own Music in the Park: Let the kids loose at the playground, bring some comfy chairs, a nice book and relaxing music

10. Before bed walk: Take the kids out on a walk before dark (or bikes) and let the stress of the day drift away. Maybe a nature walk near by. For my Catholic Members, throwing in a rosary during this Month of May (especially if you can walk to a Shrine) is a nice bonus too.

Many questions in and out of Catholic Circles have arisen over the recent FDA approved vaccine over ethical and medical issues. First off, did I get the vaccine? Yes, I got it even though my risk of severe illness is low, I felt it important to reduce risk of getting sick and spreading to others including to my high risk patients, my family and others. Not only data shows a reduction in symptomatic cases of COVID 19 by 95% with Pfizer and Moderna Vaccine, but also severity of illness. We know symptomatic cases vs asymptomatic cases are at increased risk of spread so this in and of itself would suggest potential to reduce spread. The question arose if it reduces asymptomatic cases, but we now have data from testing of Moderna recipients of a reduction even in asymptomatic cases by 63%. This could have profound impact on reductions in hospital strain over admissions and death from COVID complications. Also with the hope to reduce the number of cases long term, the chance to get a little more normalcy in life would be welcomed.
Now to some of the medical questions that have arisen. Some have been concerned about how rapid the vaccine was produced or if safety steps were skipped and this is novel technology. Yes it was rapid, but steps like animal testing were not skipped (they just overlapped phase 1 testing and a false narrative arose when this data was not published prior to the human testing). It was apid due to the world wide demand and governments putting large amount of funding into the development. Also this technology is not entirely new, it has been used in vaccine trials for cancer vaccine trials without a signal of some great harm and also used in animal viral vaccines for a number of years. Does the mRNA get into your cell and change your DNA? No the mRNA cannot get into the nucleus itself and is broken down in the cell over a couple of days, hence why you need two doses to boost the immunity enough. Does the vaccine cause infertility issues? The claim is based on an alleged similarity between the SARS-CoV2 Spike protein—which is the target for the COVID-19 vaccines made in the USA and UK—and the syncitin-1 protein found in the human placenta. Spike and syncitin-1 are completely different proteins. They share a similarity of 4 amino acids out of 1,273 amino acids in the Spike protein. Also no trial data has shown this to be a concern up to this point.
Now it has not yet been tested in pregnant women although clinical trials are going right now and overall Medical Groups generally think it is safe for women who are pregnant or plan to become pregnant (though as always discuss with your doctor). Data shows for pregnant women who do get severe COVID cases are more likely to need ICU care than nonpregnant counterparts, though in general not at higher risk for severe disease unless other comorbid conditions such as diabetes or obesity. We also have no solid data that the virus crosses plancentas or cause birth defects. If pregnant with severe COVID preterm delivery risk is higher though.
Yes there are side effects though Severe Adverse reactions were low and similar to placebo 1% and many not likely linked to the vaccine itself. Cases of anaphylaxis (acute swelling or face tongue and throat leading to respiratory distress) is rare though if prior severe allergic reaction to vaccines or components of the vaccine one should avoid it. Pain at the site of administration is most common followed by fatigue, headache, joint pains, chills and fevers (typically low grade to 101-102 range for a few). Symptoms are worse after the second dose since your symptoms are due to your immune system reaction (and hence why older patients with less robust immune systems did not have as many side effects). took ibuprofen after my second dose to ride out the symptoms that last 1-2 days myself. Main thing was fatigue, and some chills and some arm pain for me.
For Pfizer the side effects by percent:
Injection site reactions, reported by 84 percent of trial participants
Fatigue, reported by 63 percent of trial participants
Headaches, reported by 55 percent of trial participants
Chills, reported by 32 percent of trial participants
Joint pain, reported by 24 percent of trial participants
Fever, reported by 14 percent of trial participants

As far as ethical issues, the Moderna and Pfizer, confirmatory testing to proove the vaccine makes the spike protein were done using cell lines derived from an aborted fetus back in the 1970s. Although not an ongoing acquiring of cells from more abortions their use is still problematic given the source. Unfortunately these cells are used throughout many areas of research and in other areas besides vaccines such as cancer treatment, protein expression testing used to advance knowledge on pathophysiology and drug development even. This makes it hard to avoid some type of indirect cooperation. Even other areas of medicine like treatments for burn patients and dehydration were derived from immoral testing done by N**i physicians and scientists that are still used today. Sadly medicine is a mix of moral and immoral research over the years and it is hard to disentagle. For Catholics in this group, any groups from the Pontifical Academy for Life to National Catholic Bioethics Center, USCCB and my own National Catholic Medical Association have determined that receiving the vaccine is not illicit due to fairly remote cooperation with the use of these cells (and for Moderna and Pfizer does not require their use for ongoing production), and given gravity of the situation with limited alternatives. If a more licit vaccine that does not involve the use of those cell lines even in confirmatory testing were available that would be preferable. Of course everyone needs to make a personal decision based upon their conscience and medical concerns and that should be respected although I suggest you get accurate information before arriving at your conclusion. It would be admirable to avoid these cells in the future, and we are called to make known our desire for this. Writing to the makers of these vaccines is one thing an individual could do, though to be honest they will probably not be too concerned about a few people voicing a desire for alternative development. I would say a better approach may be to consider a donation to the John Paul II medical Research Institute that is trying to develop a vaccine free from any involvement of these lines and developing an ethical cell line https://www.jp2mri.org/institutes-covid19-research-prog. By not only calling the world to the truth about the inappropriate use of these cell lines, if we offer an ethical replacement that could be easily adopted, this could be our best chance for a long term change in research that is needed.

I hope this was helpful for you and again this should be a choice arrived at through personal discernment. I personally feel that the vaccine although not perfect ethically is sound medically, as well as with enough people being vaccinated could really turn the tide in surges being experienced across the US and world. Both Pope Emeritus Benedict and Pope Francis have received the vaccine as well. You should talk to your P*P or Women's Health Provider about availability in your area and your personal medical conditions to determine if it is right for you from a medical standpoint.
