Map Your Care: Illness & Grief Counseling offers support from the moment of a new diagnosis to the last breath, and beyond.

Rooted in a desire to normalize conversations around the hard stuff, Map Your Care can help you navigate it all. Map Your Care offers individual, family and group counseling for adults and seniors. Specializing in the following areas, Map Your Care seeks to open conversations about some of the toughest topics: chronic illness, new diagnosis, advanced/terminal illness, end of life, advance care planning, caregiver burnout, and grief & loss. Currently offering telehealth and limited in-home counseling.

We're excited to introduce our group therapy programs, starting January 2025! Map Your Care focuses on providing quality and evidence-based counseling for those experiencing illness, end of life, caregiving, and grief and loss. Our groups blend education, community, and skill-building to help you live a full and meaningful life at any point in your health.
