Transitional Primary Care at home is the primary focus of our company.
We are assisting patients who:
- have frequent readmissions to the hospital
- has an in-office primary care provider they can't see for weeks or months
- has multiple chronic conditions that need to be managed weekly
- doesn't have a primary care provider at all
- need us to follow home health orders

As HomeDoc continues to grow, we are super excited about how well our Palliative Program has taken off!! We have "one of", if not the most aggressive frequencies in the state, being that we are providing weekly visits or more if needed, to help manage these chronic conditions, but also adjusting meds as needed vs once a month or further out.

Adding our Psych NP to the mix only helps us to detach ourselves from other palliative programs, as the mental health piece could be hindering progress when it isn't addressed.
This is helping our patients and our accounts reduce their readmission rates!! For any hospice who doesn't currently have a palliative program, you can feel comfortable referring those to us, to then have us refer back to you the moment they are ready to admit!! HELP US HELP YOU!!
