Our continued mission is to provide 24-hour residential care to the medically fragile consumers whom are mentally ill, developmentally disabled and traumatic brain injured within the least restrictive environment. RESIDENTIAL CARE will provide a comfortable, safe and therapeutic living environment for adults whom are developmentally disabled. Such mental or physical disabilities include but are no
t limited to cerebral palsy and mental retardation arising before adulthood and usually lasting throughout life. Symptoms typically associated with this population include but are not limited to confusion, disorientation, depression, and difficulty in controlling emotional responses to everyday situations. Although individuals with these symptoms respond favorably to medical management, these individuals often need a setting that will provide assistance with improving and maintaining their daily living skills. RESIDENTIAL CARE services are available to males and females, ages 18 and older, ambulatory and, in general, possess good physical health. PROGRAM GOALS
• To provide room, board, personal care, community-living support and the necessary support in the activity of daily living to allow clients to maintain as much independence as possible within a residential settings
• To coordinate comprehensive treatment, rehabilitation and other essential services not provided by GLYNN CT. RESIDENTIAL CARE
• To provide a therapeutic environment conducive to facilitating independence and ultimately the transition to semi or independent living settings.
• To promote community inclusion and integration
• To decrease hospitalization rate
• To improve personal freedom, choice and responsibility
• To decrease or mange symptomatology (physical, cognitive impairment, behavioral and emotional)
• To facilitate a self-determining environment for all developmentally disabled consumers
SERVICES PROVIDED
Our facility will provide care for up to 6 consumers within a residential environment. Consumers living quarters are usually semi-private; however, private rooms are available for Glynn Ct. Residential Care – Developmentally Disabled Program Statement individuals unable to adapt to shared living arrangements. Generally, all clients will receive room, board, care and supervision services that include:
• Three, dietary, consultant approved, nutritious meals daily, including mid-morning coffee and evening snacks
• Laundry facilities, free and conveniently located on-site
• Housekeeping services, including room cleaning and linen changing
• Twenty-four hour, daily staffing
• Medication management and charting daily on clients physical and mental status
Personal care in GLYNN CT. RESIDENTIAL CARE is provided in accordance with the consumers’ individualized plan of service or treatment plan to assist them in performing personal daily activities. Personalized services are available in the following areas:
• Assistance with food preparation, clothing and laundry, and housekeeping beyond the level required by facility licensure (e.g., resident-required special dietary needs).
• Eating and feeding
• Toileting
• Bathing
• Grooming
• Dressing
• Transferring (between bed, chairs, wheelchair, and or stretcher)
• Ambulation and assistance with self-administered medication
Recreational activities will be available daily. They include but are not limited to board games, playing cards, book club, chess etc. Substantial effort will be put forth by GLYNN CT. RESIDENTIAL CARE staff to positively encourage clients to participate in weekly interventions.
