What is a Clinical Navigator?

Our Clinical Navigators at PAM Health Rehabilitation Hospitals of DE serve as expert guides for patients and families throughout the transition into acute rehabilitation. They work directly inside our local acute care hospitals and communities, collaborating with frontline medical staff and case management to provide education, support, and seamless coordination. Their goal is simple: act as supporters, facilitators, and advocates, connecting patients to their recovery journey at the highest-quality acute care rehab in Delaware.
