Please help us help you… we’d really love to have your voice heard!!!!

The Brain Injury Association of Wisconsin in collaboration with the University of Wisconsin Eau Claire is distributing a Needs Assessment Survey for adult SURVIVORS or FAMILY MEMBERS/CAREGIVERS of individuals that have or are experiencing brain injury. We need YOUR voice as we research the entire state looking for your experience with services for brain injury.

- survey will take about 30 mins to complete
- it is completely voluntary
- done over ten years ago, but current opinions are needed to improve brain injury services

That is where YOU come in! We plan on getting brain injury professionals input in the future.

If you prefer a paper copy, please email BIAW’s Executive Director Audrey Nelson at nelsoau@uwec.edu with your name and address.

If you have any questions while taking the survey, please contact Sarah or Jerry at the emails below.
