Kentucky waiver Home basics program

This program will help you in 5 ways:
1- case manager who will manage your case and make sure u receive all services u need

2- caregiver who will come to your home and help you with cleaning, cooking, Transportaion, and whatever you might need

3- home delivered meals (you get up to 10 meals per week, 2 meals per day)

4- adult daycare- you can go to one that has people your age, to socialize and make friends, do activities, you can do a day or 2 a week, they pick u up and drop you off, they have a nurse who monitor your health and keep up with your doctors appointments

5- supplies- if you ever need pull ups, diapers, or pads, wheelchair, walker, non medical supplies are covered in this waiver.
