Our services are tailored towards people with prediabetes or type 2 diabetes and have hypertension and hyperlipidemia. For another $15 / month we will send you the required lab tests

The regular at home lab tests include one A1c test every 6 months and one Lipid panel every year. In order to complete your lab tests you will follow these steps:

Prick your finger and squeeze several drops of blood onto the indicated circles present on the paper you will receive in your test kit. Follow the included instructions to send your kit back to our lab free of charge. Based on the results, our Care Team will evaluate your current treatment plan and make recommendations accordingly.
