As 2025 comes to a close and we welcome in 2026, we’re taking a moment to say "thank you" to the people and partners who walked alongside us this year, and made this year so special.

Caring for patients and families is truly a team effort. It happens because of dedicated clinicians, supportive staff, and the many buildings, referral partners, and community organizations who trust us to care for the people they serve. From quick phone calls and hallway conversations to bedside moments and family meetings, these relationships make it possible to provide care that is thoughtful, timely, and centered on what patients and families need most.

As we move into the new year, our focus remains on delivering high-quality, patient-centered care and being strong, reliable partners to those we work alongside, because no matter what 2026 brings, our promise remains the same: We’re With You.

As the holidays are upon us, we want to thank our community partners and remind you how grateful we are for the care you provide to patients and families every day.

This season often brings more time together for families, along with questions about comfort, support, and what the next step may be. While many families hope to wait until after the holidays to talk about hospice, care does not need to be delayed. The right support at the right time can make these moments together more peaceful and meaningful.

Our team remains available throughout the holidays to take referrals, answer questions, and walk alongside families in real time. Early hospice involvement can help bring comfort, clarity, and peace when families need it most.

The holidays can be especially heavy when you are grieving. But as many of us know, grief does not end when the decorations come down. It shows up all year long in quiet moments, family milestones, and unexpected waves.

Beginning in January, The Care Team is offering a virtual grief support group created to walk alongside those navigating loss. This 12-week series offers gentle guidance through key touchstones of grief, while creating space for connection, reflection, and shared understanding.

You are welcome to join us for the full series or attend individual sessions based on what feels right for you. There is no right or wrong way to grieve, and no pressure to share more than you are comfortable with. RSVP today.
