Happy National Rural Health Day!

Today we’re celebrating the nurses, aides, chaplains, social workers, volunteers, managers, patient advocates and liaisons who spend their days caring for patients across the rural communities we serve. They’re the ones driving the long stretches of farmland, the winding country roads, and those snowy midwestern two lane highways to make sure every patient gets the care they deserve - no matter where home may be.

We hear the stories every day… families who are grateful someone showed up even though they live miles from town, patients who feel seen and supported because a clinician made the drive, and partners who appreciate having dependable help in the most remote spots.

Our team makes that happen. They bring comfort, dignity, and reassurance to people who need it most, no matter where they live.

To our rural communities, thank you for welcoming us in. And, to our team, thank you for the heart you bring to this work.

There is a real , and we’re proud to be part of it. 💙🏡🚗

What a beautiful week! Across our service area, our teams had the privilege of recognizing the veterans who served our country.

To our community partners: thank you for welcoming us into your buildings and into the lives of your veterans. We are grateful for the chance to recognize their service and spend time with the patients, residents, and families you trust us to support.

It is truly a privilege to be a partner in care. 💙

As we recognize Veterans Day, we honor all who have served and the families who stand beside them. The men and women who have protected our country and the place we all call home.

Across our service areas, our teams are recognizing veterans through pinning ceremonies and small acts of gratitude that mean so much. They take time to thank those who have served, listen to their stories, and make sure they know their service will never be forgotten.

At The Care Team, we believe service doesn’t stop when the uniform comes off. It continues in how we care for one another, support families, and strengthen the communities we live in.

And while Veterans Day gives us a reason to reflect, honoring veterans isn’t something we do only once a year. It’s part of who we are, and part of what we do every day.

At The Care Team, "We’re With You" is more than a tagline. It’s who we are.

This week, as we celebrate National Hospice and Palliative Care Week, we’re reminded of the heart of what we do - caring for patients and families wherever they call home. 🏠

This week is part of National Care at Home Month, honoring all those who bring comfort, dignity, and compassion directly into the homes of the people who need it most.

At The Care Team, we know that quality, person-centered care takes a village... and we’re so grateful for every nurse, aide, social worker, chaplain, physician, volunteer, and partner who makes that possible. This week we're inviting our communities and partners to join us in action to:

💙 Educate families about when hospice can help.
💙 Collaborate to make care transitions seamless.
💙 Advocate for access to compassionate, home-based care for all who need it.

Together, we are One Voice for Care - united in compassion, purpose, and heart for those we serve.

💙 It’s National Case Management Week! 💙

This week, we’re celebrating the incredible case managers who lead with heart, expertise, and purpose. 👏👏👏

The 2025 theme, “Guiding Journeys,” shines a light on the important role case managers play in helping patients and families navigate some of life’s most meaningful health care moments.

At The Care Team, we are honored to partner with case managers who make every transition smoother and every patient journey more supported. You connect the dots, bring clarity in uncertainty, and advocate tirelessly for every patient’s best possible outcome. 🫶

To our hospital, facility, and community-based partners... thank you for the trust you place in us to walk beside you in care coordination. We see your impact every day, and we are deeply grateful for all that you do - and honored to support the work you do. From all of us at The Care Team, thank you for everything you do! 💙💚🤍

You never know what (or who!) you’ll see in our hospice offices. Today, it was Kiki the Therapy Goat, who was all tuckered out after visiting patients across our Indiana communities. 💙

Pet therapy plays a special role in hospice care. Animals like Kiki bring comfort, lower anxiety, and spark moments of joy and connection - especially for patients who may be experiencing isolation or loss of independence.

Whether it’s a soft bleat, a gentle nuzzle, or a shared smile between her handler and the patients, these visits remind us that hospice care is about living fully.

⚾ A hospice wish came true at the ballpark tonight! Thanks to the amazing generosity of the Lehigh Valley IronPigs, sweet Robert got to enjoy a night under the lights with his family and so many of our Pennsylvania hospice team - from our Administrator to nurses to our chaplain - all there to cheer him on. He even took the mound to throw out the first pitch!

Moments like these remind us that hospice care is about so much more than the final innings — it’s about making memories together. 💙

Thank you, IronPigs, for truly knocking this one out of the park! 🧢

🎡✨ Friday Funday at the Fair! ✨🎡

Today our Pennsylvania Hospice Team had the joy of partnering with Alexandria Manor for a carnival-style day filled with games, laughter, and plenty of fun. Our team brought the cotton candy, and even a bunny for the petting zoo. The highlight? Our very own Area Sales Manager, Drew Kessler, braved the dunk tank! 💦👏

While hospice care often carries heavy moments, we’re grateful for opportunities like this to connect with our communities, bring smiles, and create joyful memories together. ❤️

Recently, our Indiana Hospice team had the honor of helping fulfill a special wish for one of our patients who loved Corvettes. With the help of the Circle City Corvette Club he was surprised with one last ride in a bright red Corvette, just like the one he used to own. 🏎❤️

What made it even more meaningful was having his wife, son, daughter, and granddaughter right there beside him, sharing in the joy of the moment. One of our wonderful volunteers captured it all in photos so the family will always have these memories to cherish. 📸✨

Moments like this remind us that hospice is about so much more than the end of life. It’s about bringing families and communities together, celebrating what brings joy, and creating memories that last forever. 💞

We are so grateful to the Circle City Corvette Club and our wonderful Indiana volunteers for helping make this wish come true.
