A little about them: "We are a dedicated group of Nurses and Caregivers who collaborate with community-based organizations to serve the most vulnerable members of our population, particularly homebound older adults. This mission aligns closely with the goals of NPRA Sacramento and its members and associates.
Our services focus on the seamless transition of care and prevention of hospital readmission from hospitals or skilled nursing facilities (SNFs) to community-based settings. We specialize in supporting patients with complex medical needs—such as those recovering from stroke, traumatic brain injury (TBI), or motor vehicle accidents (MVA)—who may require tracheostomy/ventilator care, advanced wound management, PEG tube feedings, and other complex care needs.
Through Residential Assisted Living (in compliance with state licensing guidelines) or Congregate Living Health Facilities (CLHFs, pronounced “cliffs”), we provide a home-based environment where these patients receive evidence-based but compassionate care. Additionally, Senior Care Clinic House Calls offers medical oversight, including advanced wound care, palliative care, and primary care, delivered directly to the patient’s residence.
Founded and managed by Nurses since 2006, Care Home By RNs and Senior Care Clinic House Calls have consistently bridged the gap between home and health care, ensuring that individuals with complex needs receive high-quality, personalized care in the comfort of their homes or in a home environment."
