Concussion & Vestibular Rehab: Foundations

I just finished my third and final presentation on Balance & Dizziness 101: Stabilize Your Symptoms & Self. I partnered with The Aspen Club in Northern Colorado to make these happen and I wanted to share a framework that I spoke about during these interactive talks.

Your brain puts together information from 3 main systems to stay upright and moving (and ideally symptom-free!). If one or multiple of those systems don’t give accurate or reliable information, your brain A) spends extra energy trying to interpret it and B) can get confused, which leads to a symptom.

Above, you’ll find brief descriptions of what each system does. This is the basis for concussion PT / rehab: exploring these systems to find the root cause of your persisting symptoms.

Then, isolating and strengthening that system.

Then, integrating that system into progressively harder activities that get you back to your life!

Concussion recovery: understanding your thresholds! Performance versus Symptom!

“How much should it hurt?”

“How much should I push things?”

My answer is: it doesn’t need to hurt to make progress. And a little bit is okay (a 2/10 on a 10-scale).

But above all? Live to train another day. If what I do today is going to compromise my ability to do it again tomorrow, then dial it back. There are exceptions to this, but in general it’s better to be able to show up and train something every single day as opposed to being so brain-sore that we don’t want to do it again tomorrow.

This is where understanding our performance threshold versus our symptom threshold comes in handy. Training at your performance threshold is doing something hard enough that it challenges your ability to do it. In concussion rehab, that’s often your eyes’ ability to hold the target stable & steady, or your ability to balance stable & steady, or to put it all together when we layer dynamic and cognitive challenges.

Your symptom threshold is when you continue to push hard enough that it worsens or causes symptoms.
