The South bears the highest burden of HIV in the U.S. — accounting for 52% of new HIV diagnoses in 2022 despite being home to just 38% of the population.

Key challenges:
🔴 Lowest PrEP-to-Need Ratio in the nation — only 12 PrEP users for every new diagnosis.
🔴 Stark racial disparities: Black Americans made up 48% of new diagnoses but just 23% of PrEP users in 2024.
🔴 Persistent barriers — poverty, lack of Medicaid expansion, rural access gaps, and pervasive stigma.

Now, with recent threats to federal HIV funding and research, progress toward ending the epidemic in the South and in the US is at risk.

People with HIV with persistent viremia and adherence challenges to oral antiretroviral therapy can achieve viral suppression with long-acting cabotegravir/rilpivirine (LA-CAB/RPV). In CID, Pamela Pei, PhD, and colleagues project the impact of delaying LA-CAB/RPV implementation while awaiting trial data.
