The goals of CC4C are: To make sure children are raised in healthy, safe, and nurturing environments To strengthen families through home visits and personal phone calls To connect families to programs, services, and resources which meet their needs To introduce parent support programs when available To encourage and support parents in being their child’s best advocates To support children in reaching their developmental potential Eligibility: Children birth to five years who are at risk for developmental delay or disability, long term illness and/or social, emotional disorders Patients Who May Benefit from CC4C: Those with special health care needs Those in foster care and in need of a medical home Those exposed to environmental stressors
