So should you purchase one? Only you can answer that question. I know everyone couldn't make the conference in San Diego this year, so I hope this video offers information to think about if considering a new TMS machine.

*Know what you are paying for*
There are multiple options when it comes to accelerated TMS. SAINT is the most publicized protocol, but also the least accessible due to few locations and the cost of service (20k-50k). Due to the great marketing associated with the name, there are providers not being clear about the services they provide. Outcomes are likely to be good whether you do the proprietary protocol or not, but patients should still be informed of what they are purchasing.
There are very few insurance providers that cover accelerated treatments. If you're interested in maximizing your chances of success with TMS, protocols that are able to utilize functional connectivity MRIs may be what you're looking for.
In my opinion, neither a functional MRI or a standard MRI (structural) offer providers additional data for mapping. It might be easier to locate/visualize the motor cortex, which can then be used for an algorithm to locate the DLPFC. Technically though, a TMS provider could locate the motor cortex without the MRI.

Accelerated TMS is a protocol for depression that significantly reduces depression over the course of just a few days. There are no medications involved.

- Insurance does not cover accelerated TMS.

- Traditional TMS is covered by insurance, but takes 6 weeks (30-36 sessions, one session per day for 6 weeks) to complete.

- BOTH traditional and accelerated protocols are very effective for treating depression (over 70% experience positive effects)

The effects of TMS can last for months, years, or the rest of a person's life. There are no official guidelines regarding any of these questions that I am aware of. Maintenance treatments are something that need to be customized between patient and provider. As time goes on I'll be looking to discuss more on this in future videos.

Anyone who says there are no side effects to transcranial magnetic stimulation is a LIAR. You can get away with saying there are "virtually" no side effects, but in an honest discussion about the risks and benefits of TMS, tenderness, headache, and fatigue need to be mentioned. The 3 common side effects typically happen early on in treatment as people get used to the treatment. The "TMS Dip" is the 4th side effect that can happen.

In this video I answer a frequently asked question about the cost of TMS therapy for depression, anxiety, OCD, and other conditions.

Unfortunately, only TMS for depression is covered by most insurances. In rare cases, some insurances in the United States will pay for TMS for OCD. The amount of coverage for OCD might increase over time. Accelerated protocols, anxiety, Tinnitus, and other conditions are not reimbursed via insurance. Rates may vary depending on the amount of providers in the area (competition) and availability.

There are new and very effective treatments out there for depression. If you are a patient or you have a patient not entering remission from antidepressants, there is a good chance a TMS provider is near you. Don't let your patient be the one to ask you about TMS for depression. You should be bringing it up to them.
