Adult Nurse Practitioner – Concierge House Calls

Tired of seeing 30+ patients a day and practicing “assembly line” medicine with little time to think?

At A+P House Calls, we built something different—because healthcare needed to change.

This is not a traditional clinic role.
This is an opportunity to practice real medicine.
* In-home, concierge-level care
* Time to create thoughtful, comprehensive care plans
* Manage everything from acute visits to complex chronic disease (CHF, diabetes, weight loss)
* Perform full H&Ps, diagnose, treat, and prescribe independently
* Provide wellness care including HRT, TRT, OCP, peptides, and GLP-1 management
* Perform phlebotomy and IV therapy
* Interpret labs and imaging; coordinate and delegate care

You will have the opportunity to:

* Build your own patient panel
* Develop real relationships with repeat patients
* Function in a true primary care role
* Grow into leadership positions with increased compensation

We are looking for providers who are:
Highly autonomous and clinically confident.
Experienced (5+ years, urgent care/primary care/ER preferred).
Ready to step away from burnout and into purpose-driven medicine .

Travel: Eastern Queens, Nassau and Suffolk County (primarily North Shore, with some South Shore)
Schedule: Flexible | 35 hours/week preferred + every other weekend
Compensation: Competitive with performance-based growth
