Licensed Practical Nurse:
Job Type: Part-time 24 hours weekly
Job description:
Overview: We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality client care in an Assisted living. As an LPN, you will play a crucial role in supporting Clients health and well-being through effective communication and collaboration with healthcare professionals. This position has no weekend requirement!
Responsibilities:
Provide direct client care, monitoring vital signs, and assisting with daily living activities.
Conduct patient assessments and document medical histories accurately in compliance with HIPAA regulations.
Collaborate with registered nurses and physicians to develop and implement individualized care plans.
Perform procedures such as wound care, foot care, PICC line dressing changes and specimen collection as needed.
Maintain accurate medical records and documentation using EMR/EHR systems.
Participate in care planning and case management to ensure continuity of care.
Uphold infection control protocols and maintain a safe environment for patients.
Experience and requirements:
Long-term care, assisted living settings, group home setting experience
Working with individuals with developmental disabilities or dementia care
REQUIRED current and unrestricted LPN License issued by the state of MN Board of Nursing.

Direct support professional:
Job description:
We are seeking compassionate and dedicated individuals to join our team as Direct Support Professionals. We have dayshift and nightshift available. Full or part time. As a Direct Support Professional, you will provide essential care and support to individuals with developmental disabilities. Your role will involve assisting with daily activities, ensuring their well-being, and promoting their independence.
Duties:
- Provide direct care and support to individuals with developmental disabilities
- Assist with activities of daily living (ADLs) such as bathing, dressing, and grooming
- Administer medications according to prescribed guidelines
- Monitor and document individuals' health status and report any changes to the nurse
- Assist with meal planning, preparation, and feeding if necessary
- Engage individuals in recreational activities and promote social interaction
- Provide transportation for community outings and appointments
Requirements:
- Previous experience in patient care or working with individuals with developmental disabilities is preferred
- Ability to lift and assist with transferring individuals as needed
- Knowledge of HIPAA regulations and ability to maintain confidentiality
- Familiarity with medication administration procedures
- Understanding of hospice care principles is a plus
- Knowledge of dementia care techniques is beneficial
- Valid driver's license and reliable transportation
