This month our success story has not returned home yet but is continuing to work towards going home.William Carlton use to live alone and was independent with all his Activities of Daily Living and mobility tasks. When William was admitted to the Harlan Health and Rehabilitation Center,he was very debilitated.He had a severe stroke which affected his overall strength ,ability to ambulate,ability to perform bed mobility tasks,decline in his activity tolerance,decreased ROM in his upper and lower body, decreased level of cognitive ,level of alertness,impaired safety awareness,impaired receptive and expressive language skills,and his ability to express his wants and needs. Through very intensive skilled therapy services of occupational,physical,and speech therapy, Mr. Calton was able to increase his level of independence. Mr. Calton worked with Speech Therapy to increase the ability to let his wants and needs be known,increase his receptive and expressive language skills, and his overall cognition. He worked with Occupational Therapy and Physical Therapy on increasing the following: overall strength;standing and balance;increasing his ROM in his upper and lower body;improving his safety awareness;his ability to transfer using a rolling walker with minimal assist;increased his independence in his ambulatory to CGA-minimal assist with a rolling walker;improved his ability to maneuver in/out of bed with minimal assistance from staff;able to perform bed mobility to CGA;and increased his upper body dressing and grooming to CGA- minimal assist from staff. He is able to move around the facility in his wheelchair. Mr. Calton made substantial gains in therapy and toward his goal of going home. We are so proud of what he has accomplished.
