When you come in for your first appointment with us, you’ll need to bring a few important items with you:

✅ Personal identification, like a driver’s license or student ID
✅ Insurance card
✅ Payment method for insurance copay, if required for your policy
✅ New patient paperwork
✅ Medication list (optional)

We need these to register you in our system and record information for our clinical team ahead of your consultation.

Double check this list ahead of your first visit to prepare for check-in, and give us a call if any questions come up!
