If you are insured by ANTHEM MEDICAID, your MCO has changed to either Humana Medicaid or UHC Medicaid as of 01/01/2025. Please let us know if you have Anthem Medicaid when you come in for your next appointment.

If you don't have your new insurance information from Humana or UHC, we will gladly help you find it at your next appointment!
