At the appointment, you can sign the release for UT to obtain your medical records, stored with Privia and/or Clary.

As a refresher on the medical record topic....For patients who were seen prior to 6/26/23, your records have been transferred to Clary Document Management. You may request copies of your records by filling out the form at: https://medicalrecordcustodian.com/friendswood-gynecology/

For patients who were seen after 6/26/23, Privia maintains your records in the new Privia system. These records can be requested by emailing: medicalrecords@priviahealth.com. The Privia system should have both the old records from prior to the 6/26/23 migration and after. The records that are with Clary will ONLY be those prior to 6/26/23.

Hilarious! In my excitement, I did not clarify.............DR. ERICA ROBERTS is joining UT Health! I'm super excited! (My kiddos would be embarrassed by my amateur FB use!!!)

If you contact either UT Health office right now, they do not have a set schedule quite yet to get a patient on the books, but that will be finalized in the next couple weeks!

Good morning! I have exciting news! I have officially signed on to start with UT Health mid July and will be practicing locally at their Memorial Hermann Southeast and Friendswood locations! I plan to continue gynecology only and I would love to be able to continue taking care of my amazing patients that I have seen over the years!

Hello everyone,
As many of you know, our office has permanently closed. We finally have an update on medical records! Thank you for your patience during this transition.

For patients who were seen prior to 6/26/23, your records were in our old “legacy” system. These records have been transferred to Clary Document Management. You may request copies of your records by filling out the form at: https://medicalrecordcustodian.com/friendswood-gynecology/

For patients who were seen after 6/26/23, your records were in the new Privia system. These records can be requested by emailing: medicalrecords@priviahealth.com. The Privia system should have both the old records from prior to the 6/26/23 migration and after.

The records that are with Clary will only be those prior to 6/26/23. There were a few random charts that did not completely migrate from the legacy system to Privia. If you feel that your chart is incomplete with Privia and you were seen in both systems, feel free to reach out to Clary using the link above to retrieve your records from the legacy system.

As for updates on the providers:
• Dr. Shamburger has accepted a position that does not offer patient care.
• Dr. Roberts is still considering her options.
• Erica Mitchell is seeing patients at Houston Methodist Clear Lake Ob/Gyn office beginning 7/1/24.

Update on medical records:
The records have been transferred to Clary Documents storage. Clary is now integrating the data into their system. As soon as that is done, they will provide instructions to pass along to patients as to how to access/request records. Thank you for your patience.

Hello everyone,
I know y’all have been patiently waiting for updates. Here is the latest:
Dr. Shamburger will be transitioning to a non-patient care role and will no longer be seeing patients.
Dr. Roberts and Erica Mitchell are still reviewing options. Nothing has been decided yet.
The office is now closed. If you requested your medical records, we are hoping to have those requests completed by Monday. If you haven’t requested yet, there is no rush. They have to be maintained for 21 yrs, so you have time. 🙂 We are in the process of setting up an account with a company for medical records storage, but it will take some time. We are expecting this to be completed by mid-May. Remember, if you haven’t had any procedures with us in the last couple of months, there is a low likelihood that records are even necessary for a new Gyn. Once the account is set up and ready to receive requests, we will post here and on our website, as well as sending out emails with instructions.
Again, thank you all for your trust over the years and your patience in the last month as we have made this challenging transition.

As many of you have heard, we are closing the office next month. The last day that we will see patients will be April 8. We are happy that we had the opportunity to be with our patients as they started or grew their families, and we have enjoyed getting to know our patients over the years.

We have had many requests for medical records over the past few days. Currently, we can get those to our patients one of two ways--paper print out that must be picked up or via fax. We do not have a way to securely transmit the records to personal email addresses. The paper print out will have a small fee assessed depending on the number of pages that need to be printed. If you have been seen after 6/26/23 in our office, you will be receiving a separate message from Privia with instructions on how to obtain your medical records from them.

Another option is to transfer the medical records to another physician's office. These are the easiest requests to process. You can print out a medical release of information form from our website and then email to officemanager@friendswoodwomen.com. We can then send your records to your new physician.

In addition, we are in the process of interviewing medical record storage companies who will take over the storage and processing of requests after May 1. These companies typically have a secure way to transmit records to personal email addresses. Once we have decided on a company, we will send out another email (and post on FB) with instructions on how to request records.
