Why can a panic attack feel like a heart attack?

A panic attack can trigger your body’s fight or flight response, including a rush of stress hormones like adrenaline. That can create very real physical sensations such as a fast heartbeat, chest tightness, shortness of breath, sweating, and nausea.

It can feel even scarier because the brain’s interoception system, which helps you notice and interpret internal body sensations, can become highly focused on heartbeat and breathing. The insula is one region involved in this body awareness. When anxiety is high, normal stress sensations can be interpreted as danger, which escalates fear.

What DSM-5 says about panic attacks:
A panic attack is an abrupt surge of intense fear or intense discomfort that reaches a peak within minutes. During that surge, 4 or more symptoms occur, such as:
Body symptoms: • Palpitations or pounding heart • Sweating • Trembling or shaking • Shortness of breath or feeling smothered • Feeling of choking • Chest pain or chest discomfort • Nausea or abdominal distress • Dizziness, lightheadedness, or feeling faint • Chills or heat sensations • Tingling or numbness

Mind and perception symptoms: • Feeling unreal or detached from yourself • Fear of losing control • Fear of dying

How long does a panic attack last?:
Panic symptoms usually peak within minutes. Many last about 5 to 20 minutes, sometimes up to about an hour.

Panic attack and heart attack can overlap:
Both can include chest discomfort, shortness of breath, sweating, nausea, and lightheadedness. That overlap is exactly why it can be hard to tell them apart at home.

More common with panic attacks:
These clues lean panic, especially if you have had similar episodes before. • A sudden surge of intense fear or dread • Symptoms peak fast, often within minutes • Tingling or numbness, chills or heat sensations • Trembling or shaking • Feeling unreal or detached • Fear of dying or fear of losing control

More concerning for a heart attack:
These clues lean medical, especially if they are new for you. • Chest pressure, squeezing, fullness, or pain that lasts more than a few minutes, or goes away and comes back • Discomfort spreading to the arm, jaw, neck, back, or stomach • Shortness of breath with or without chest discomfort • Cold sweat, nausea, lightheadedness • In women, symptoms can also include unusual fatigue, upper back pressure, or upset stomach

Important safety note:
If chest symptoms are new, intense, different than usual, or you are not sure, treat it as medical first and call 911 or seek urgent care.

On this Thanksgiving day, I want to say thank you for being here and for the ways you continue to care for yourself.

Holidays can hold gratitude, stress, joy, tension, and everything in between. All of it is normal. All of it is human.

If today feels overwhelming or emotionally full, here are three small practices that can bring your nervous system back to center:
1. One honest moment of gratitude
Take 30 seconds to notice one thing that feels genuinely supportive right now. A warm drink. A quiet corner. A safe place to breathe. One is enough.
2. One boundary you can keep
Choose one small boundary that protects your peace, like stepping outside for a few minutes when things feel intense or keeping a short window of quiet before bed.
3. One ten-minute digital pause
Put your phone in another room for ten minutes. Breathe slowly, stretch, sit in silence, or listen to calming music. Give your mind a chance to settle.

My name is Anand Vyas, Ph.D., NCC, LPC also known as Dr. V. I'm a fully licensed, experienced (10+ years) and down-to-earth professional who is committed to helping you.
