Grateful to Help Another Family

I recently had the privilege of assisting a wonderful gentleman in finding the perfect memory care community in La Habra. After just a few days of touring various options with his two sons, he chose a community that met his care needs, location preference, and budget—and he couldn’t be happier.

At Transitional Care Solutions, we prioritize Care Needs, Location, and Budget when guiding families through the process. For seniors, the right fit depends on their medical diagnosis and level of care required, whether that’s Memory Care, Assisted Living, or Independent Living.

In this case, due to his dementia diagnosis, memory care was the ideal choice to provide him with the safety and support he needed. Helping families navigate these decisions is truly rewarding! Memory care is a specialized type of residential care designed to meet the unique needs of individuals with memory-related conditions, such as Alzheimer’s disease, dementia, or other cognitive impairments. These communities provide a safe and structured environment with specially trained staff to support residents in maintaining their quality of life while addressing challenges like memory loss, confusion, or behavioral changes.

Key features of memory care:
• Specialized Programs: Activities and therapies tailored to stimulate memory, promote engagement, and reduce anxiety.
• Safety Measures: Secured facilities to prevent wandering and ensure residents are in a safe environment.
• Trained Staff: Caregivers trained in dementia care, focusing on patience, understanding, and de-escalation techniques.
• Personalized Care: Assistance with daily tasks like bathing, dressing, medication management, and meal preparation, all adapted to the resident’s cognitive abilities.

Memory care communities aim to provide not only safety but also dignity and enrichment for residents, offering peace of mind to their families.

Helping families find the right place for their loved ones is what we do best! This wonderful lady is now happily settled in an assisted living community in Orange County, one of many placements we’ve had the privilege to assist with. Her family couldn’t be more relieved and thankful for a smooth transition. Grateful to have been part of their journey and to witness the peace of mind this placement has brought.

Finding My Purpose: The Journey to Becoming a Senior Placement Expert

In high school, I met a remarkable lady in her 80s at my church. She was a widow with few friends or family nearby, and I quickly became a supportive friend to her. Our time together was simple yet meaningful—we talked about life, went for walks, and occasionally indulged in bundt cakes from Arby’s. I helped her with her garden and took her to doctor’s appointments, forming a bond that left a lasting impression on me.

As high school graduation approached, my visits became less frequent, and over time, we lost touch. Yet, the love and memories we created inspired me to pursue a career in nursing home administration. I wanted to positively impact the lives of elderly individuals, the dedicated caregivers, and the families who cared for them. While being a nursing home administrator was rewarding, I felt there was something greater waiting for me.

That opportunity came unexpectedly. I found myself in Texas, unemployed and searching for direction to provide for my family while fulfilling my desire to make a meaningful difference. One afternoon, a feeling prompted me to call a friend running a senior placement business in Orange County. I remember that call vividly; it was a Monday, and he shared that North Orange County had just opened up due to a partner leaving. In that moment, I decided to take the leap and start my own journey as a senior placement specialist.

Since making that decision, my life has transformed in ways I never anticipated. I now balance my time between my family and helping others navigate the complexities of the senior living sector. The joy I feel in my work is immeasurable. I love building long-lasting relationships with families and their loved ones, guiding them through the process of finding the right senior living options—be it assisted living, board and care, or memory care.

There’s nothing more fulfilling than receiving referrals from families in need of assistance. I take pride in alleviating their stress and confusion, empowering them to make the best decisions for their loved ones. Best of all, my services come at no cost to them. Walking alongside families each step of the way, I find purpose in helping them navigate this important transition, knowing I am making a real difference in their lives.

Conclusion

Reflecting on my journey, I realize that every step—every conversation, every walk—has led me here. I am not just a senior placement expert; I am a friend, a guide, and an advocate for families facing challenging decisions. And I wouldn’t have it any other way.

Senior Care Transition and Options Chart…

This chart provides a clear view of the care transition process, from the highest levels of care—such as a hospital—to lower levels of care, like Board and Care Homes or Assisted Living facilities. While emergency hospital visits are never pleasant, and we can’t predict when they’ll occur, we can prepare in advance to manage them effectively.

At the hospital, our ultimate goal is to return home, where we feel safest and most comfortable. However, it’s not always feasible to go home directly after an emergency, especially if around-the-clock care is required.

If you find yourself needing to navigate the next steps after a hospital visit, rest assured that we are here to assist you through every stage of the process.
