And often? People are waiting longer than they need to, missing out on care and comfort measures to increase the quality of life of their loved one.

📣 November is National Family Caregivers Month!

We know how much falls on you when you're the primary caregiver to a loved one.

We've had the privilege to work with many, many caregivers over the years and we know what some of the most common questions, biggest challenges, and most confusing topics of caregiving can be.

Our Paradigm Health Hospice and Palliative Care team in Fort Wayne got into the Halloween spirit today! We had plenty of laughs, a few scares, and lots of treats, all while continuing to care with heart.

We were honored to join Marquette Manor and Greenwood Village South in supporting the 17th Annual “A Round to Remember” Golf Tournament benefiting the Alzheimer’s Association.

Sarah Christian represented Paradigm Health on the course as a hole sponsor, showing our continued support for families impacted by Alzheimer’s Disease and helping raise awareness about the importance of Alzheimer’s care and research.

Together, we drove change and hope for those affected.

It has always been an honor to serve those that have served our country. Our partnership with the We Honor Vets program as a Level 4 Partner means that our veterans receive exceptional care tailored to their needs.

In addition to coordinating care with the VA and other organizations, we work to pair veteran volunteers with veteran patients, and to connect families with other community resources.

Spiritual Care Week is a time when we honor our chaplain team. Their heartfelt commitment to providing spiritual and emotional support to those that request it is unmeasured and selfless. 💚

For a lot of patients -- even those that have not been traditionally spiritual throughout their lives -- receiving hospice care can highlight some spiritual questions, needs and desires. When this happens, part of our comprehensive care includes support from our incredible chaplain team. Our chaplains have dedicated themselves to the care and well being of others, and we are so grateful for the ways they walk with our patients through it all.

To our incredible, compassionate, gifted chaplains: Thank you for the ways you show up every day to support not only our patients, but the entire Paradigm Health care team as well. The contributions you make cannot be overstated. We are so grateful for you.

This past month, the Paradigm Health team came together for our Fall Celebration of Life service, a heartfelt time to remember, honor, and celebrate those we’ve had the privilege to care for. These gatherings bring together families and caregivers to reflect on cherished memories and celebrate lives well lived.

Through our Bereavement Program, we continue to support families for up to 13 months following the loss of a loved one. That support includes regular mailings, phone check-ins, remembrance events, and access to counseling with our bereavement team and chaplains.

At Paradigm Health, our commitment is simple: to walk alongside families through every stage of healing, offering compassion, understanding, and community when it’s needed most.
