Through the use of a family-centered care approach, Heal At Home nurses and home health aides work closely with the family to build confidence, and create and implement the right care plan. Care that improves the quality of lives and fosters independence. Care that gives patients and their families dignity, respect and hope. The safety of our patients and caregivers is of ut
