Heal at Home Heal at Home focuses on family-centered care to give patients dignity, respect, and hope. Care that supports family involvement.

Family Centered Focus:
Through the use of a family-centered care approach, Heal At Home nurses and home health aides work closely with the family to build confidence, and create and implement the right care plan. Care that improves the quality of lives and fosters independence. Care that gives patients and their families dignity, respect and hope. The safety of our patients and caregivers is of utmost importance to us. We educate all of our staff on how to recognize, resolve and prevent dangerous situations for our patients and employees. Heal at Home is a Medicaid home health provider.

Dear caregivers:

As you know, Indiana transitioned Level of Care Assessments to Maximus in July. While FSSA reported that Maximus was responsible for reaching out and scheduling Level of Care Assessments for your yearly reassessment, we have had several families report that they had to reach out to Maximus or have their Waiver Case Manager reach out to complete their assessments on time.

Please contact your Case Manager and ask when your loved one's yearly Level of Care Assessment is due. If it is in the next three months, please reach out to Maximus and have your Waiver Case Manager reach out as well.

We want to make sure all of our families have all needed paperwork submitted in time for their yearly waiver renewal.

Indiana now requires CPR certifications to be completed in person. If our team reaches out to let you know you need to recertify, you are welcome to complete your training through Healing Hands/Heal At Home or through another provider. Simply email us your updated CPR certificate!

FSSA created metrics that increase audits when a company’s caregivers miss 15% or more of their clock ins/outs. These audits draw staff time and resources that should be going towards our patients and caregivers. If you clock in and out daily, thank you. If you don’t, please set timers on your phone as a reminder. It’s an important part of being a paid caregiver and contributes to a better community.
