If you are having a medical emergency, please call 911 or go to the closest emergency room for care. In urgent situations after hours, our patients can call the center number and be connected with our answering service. They will contact the provider on-call for you who will call you back. PREPARING FOR YOUR APPOINTMENTS

New Patient Appointment Arrival Time
Please arrive at least 30 minutes before your scheduled time to complete the registration process. Bring your driver’s license/government ID and insurance cards. If you don’t have insurance, bring proof of household income: tax forms, paycheck stubs, bank statements or MDHHS eligibility forms. Return Appointment Arrival Time
Please arrive at least 15 minutes before your appointment time to sign in and do any updates in your information that may be needed. Bring a copy of your driver’s license/government ID and insurance cards. Late Arrival
We try our best to see you at the time of your scheduled appointment. If you don’t arrive on time, you may need to wait to be seen (we would have to consider you a “walk in”) or could be rescheduled. Cancellations
If you cannot keep your appointment, use the patient portal, or call 313-561-5100 to cancel. Please cancel at least 24 hours before your scheduled appointment if you can, so we can give another patient a time to come in.
