Summit View Physician Services provides in-home care for geriatric and homebound patients. We're proud to utilize a concept called, "continuum of care".

Our highly trained visiting clinical staff can perform most of the same services that a patient would receive in a doctor’s office but in the comfort of their own home. This approach integrates various systems and levels of care that help our practitioners guide and track patients' health over time. More specifically, we do this by using a comprehensive array of health services that span all levels and intensity of care. Summit View Physician Services focuses on providing high-quality and compassionate treatment and care in the safety and comfort of a patient’s home. Our ability to conduct tests and follow-up visits in the home, makes the health care treatment process much more practical, convenient and more comfortable for care-givers and patients alike.
