Coding doesn't stop when you've selected all the correct HCPCS codes. Coding only describes to the payer what you are providing, but it does not unlock payment. You need to meet the payers policy coverage for the device and document medical necessity stating how the patient will benefit from the codes you selected.

Fact! There is a common misperception that orthotist/prosthetist (O&P clinician) notes aren’t considered during formal claim reviews.

Your clinical notes do matter—they’re an important part of the patient’s medical record. But here’s the catch: they can’t stand alone to prove medical necessity. Clinicians should thoroughly write detailed notes that substantiate the physician’s documentation.

Let’s keep the records clear, consistent, and complete! 💪

Clinical documentation for custom fabricated devices continues to face increased scrutiny. Simply stating that a custom device (e.g., socket, AFO, back brace) was provided isn’t sufficient. The medical record must clearly demonstrate that a custom fabricated device was produced. This could include a detailed note describing the fabrication process, including the materials used, or uploading a central fabrication order form directly to the patient’s chart.
