The Medicare GUIDE Model (Guiding an Improved Dementia Experience) is a nationwide voluntary initiative aimed at supporting individuals with dementia and their caregivers. It began on July 1, 2024, and will run for eight years.

To qualify for the GUIDE Model, beneficiaries must:
1. Have a clinician-confirmed dementia diagnosis.
2, Be enrolled in Medicare Parts A and B (not in Medicare Advantage or PACE programs).
3. Not have elected the Medicare hospice benefit.
4. Not be a long-term nursing home resident.

Non-certified home care providers must find the nearest approved federal provider in your county(ies) and apply with them.

The model aims to improve dementia care by testing the efficacy of specialized care, including support through care management fees and respite services.

Again, this is a demonstration program which is running for 8 years (started in 2024). CMS will analyze outcomes and share them with the public.
