Hours of Operation/Policies/Consent
*Monday - Friday (8am-5pm)
*Appointment Time/Tardiness- BY APPOINTMENT ONLY. I see one client at a time please do not be too early or late but on time for your appointment. Please be respectful that the appointment times are scheduled and cannot exceed beyond the stated time unless approved by me.
*Cancellation Policy-A 24-hour notice is required for cancellation of an appointment, or you will be charged in full for the appointment.
*Sickness- Massage bodywork is not appropriate care for infectious or contagious illnesses. Please cancel your appointment as soon as you are aware of an infection or contagious condition. If it is within the 24-hour notice, the cancellation fee may be waived.
*Understand-Because massage therapy should not be performed under certain medical conditions, you have stated all known medical conditions, and answered all questions honestly. You understand that massage therapist are not primary care providers, and is not a substitute for medical examination, diagnosis or treatment. You will keep me (Therapist) aware of any changes to medical profile, you will immediately inform the therapist if you experience any pain or discomfort during the session, and there shall be no liability to the part of the therapist if you fail to do so. You understand that you may be required to receive clearance from your primary care provider before receiving massage.
*Sexual Misconduct- You understand that a practitioner's touch and the manner of communication between therapist and client are never intended to be sexual in nature. You understand that any sexual comments or remarks or advances made by you (the client) Will NOT be tolerated and are grounds for immediate termination of the session. You understand if the therapist has to end the session for sexual misconduct that you are responsible for the full amount due.
*Privacy policy- You agree and consent to assessment and treatment. You understand that your personal health information will be collected. You understand that all information that you provided will be kept confidential unless required by law. You understand and consent that your medical information may be shared by the various care providers involved in your care and treatment.
