I haven’t had to worry about weight most of my life due to a generally healthy metabolism & staying active.

But in my early 40’s, everything changed. I was a new mom, trying to run a business and be the best doctor I could. I had slowly managed to gain 30 pounds, and finally became sidelined with kidney stones.

This led me to understand my genetics.

Once I ran my genetics into the GeneRx report, I realized that while I “thought” I was eating healthy and exercising the right way - I had it all wrong! I was overdoing protein and running too fast for my genetics.

All I did was start eating the right amount of protein, change the way I was exercising, and take a few key supplements to support my variant stress and inflammation genes.

I kid you not, 2 months later, I was down to my healthy body weight! When we give our body what it needs, it will come back into balance.

The truth may feel fuzzy because we need to get our definitions straight.

Perimenopause and menopause are distinctly different stages with different physiology.

Perimenopause: the months and years leading up to menopause where ovarian function declines and menstrual cycle variation begins. Ends 12 months after the last menstrual period. This is a time of hormonal variation.

Menopause: the permanent cessation of your menstrual cycle, starting 12 months after last menstrual period. This is a time of low hormones.

HRT may be prescribed to stabilize hormones in perimenopause - not necessarily to support low hormone levels, as may be indicated in menopause.

But, perimenopause and menopause are about so much more than low estrogen.

If we can support cortisol, inflammation, muscle health, cardiovascular health, bone health and sleep in EARLY perimenopause, this will set you up for less problems in late perimenopause and menopause.

Understanding your genetics helps you to dial in:

Many of us know those who did great on a keto diet, and those who did terribly! Maybe they gained weight, looked puffy, had redness in the face, or experienced joint pain.

What might be good for one person is not necessarily good for another because we all have a unique design.

Because we live in an environment full of toxins, are being exposed to novel viruses, and are eating toxic foods, we can no longer get away with just “eating healthy” and guessing about how to take care of ourselves.

Thankfully, we now have the technology to understand our genetics in an integrative way, giving us an edge on how to care for ourselves.

When we understand specifically how much protein we need, how sensitive we are to carbs and saturated fats, the right kind of exercise for our bodies, how our detox pathways work, whether we need to support our bodies production of stress hormones, inflammatory cytokines, and/or sleep hormones, whether we need to support our bodies metabolism of estrogen, etc. then, we give our bodies the input it needs to maintain homeostasis.

I’d like to continue the conversation on HRT and perimenopause from my post last week. There are many other symptoms of perimenopause (fatigue, weight gain, insomnia to list a few), but the point is that HRT is only shown to be helpful for a handful of perimenopause symptoms; yet, so many women are looking to HRT to resolve all of their symptoms.

Naturopathic medicine has a multitude of strategies for addressing perimenopausal symptoms and concerns WITHOUT always requiring the addition of hormones.

Why the confusion around HRT then? The 2019 Lancet meta-analysis on HRT did NOT take into account individual genetics. So, you have a pool of women, some of whom have a decreased genetic risk of breast cancer, some of whom have an increased genetic risk of breast cancer, and when combined, the risk appears to be neutral.

The good news is that you can learn about your estrogen metabolism by having your genetic data evaluated!

In the interest of “doing no harm,” I evaluate genetics on all of my patients before creating treatment plans involving hormones. Understanding your individual genetics is key to making educated decisions about your health.

I thought I was doing all the "right" things.

You know -
⚡️eating all the protein
⚡️intense workouts
⚡️snacking regularly

But, despite these things I just continued to gain weight postpartum.

After 7 years, I had put on about 30 pounds.

I ran my genetics and discovered.

⚡️I didn't need much protein! I just needed adequate protein - far from the 40 g per meal that many are pushing
⚡️I needed to moderate my workouts - too much intensity drives inflammation for me
⚡️Snacking was not my friend - it was causing too many blood sugar spikes

Once I adjusted my lifestyle - fairly effortlessly - the 30 pounds came off very quickly.

I gave my body the right inputs (the exact amount of protein, consistent and moderate exercise, and periods of fasting) - and my body responded!

The answers are in your genetics.

We are in an era of personalized medicine. Rather than looking to people whose bodies you admire, you might just look deeper into yourself (your genetics).

Because those women with great bodies may have just figured out what works for them - but they don't know what works for YOU unless they are looking at your individual genetic code.

Myth - Perimenopause = need for HRT

Truth: the most common symptoms of perimenopause are hot flashes, insomnia, fatigue, mood swings/anxiety, and weight gain.

It’s easy to assume symptoms like low libido, joint pain, brain fog, and migraines are likely related to perimenopause because this is the recurring message being perpetuated on social media. However, these are also symptoms of systemic inflammation and likely elevated cortisol.

Inflammation has many causes, including pathogens, toxins, and lifestyle factors.

Cortisol levels do rise in perimenopause, and are known to cause low libido and brain fog; however, the treatment here would be to address cortisol, rather than estrogen

Testing for and addressing cortisol and inflammation is important, alongside understanding estrogen levels in perimenopause.

This approach recognizes that each woman's body is unique and may require a different treatment plan, emphasizing the importance of personalized care in addressing health concerns.

You are not designed exactly like anyone else, thus your treatment plan for perimenopause should not be one size fits all!

While browsing Facebook a few weeks ago, a post about Hormone Replacement Therapy (HRT) in a mom thread got my attention. A woman in her mid-30s, who thought she was undergoing perimenopause because she had low libido, joint pain, brain fog, and migraines, was crying out for help to find a doctor who would prescribe HRT. She lamented that her doctor wanted to work on lifestyle factors while prescribing a few natural supplements.

The comment feed was filled with support from other women, providing links to telehealth doctors who they were confident would prescribe the medication. I restrained myself from commenting-I would have offered the controversial take that her doctor was doing the right thing, and that she should steer clear of doctors who generously prescribe hormones. I knew this was a much larger conversation than could be addressed in a Fb thread.

The primary indications of HRT in perimenopause are hot flashes, vaginal atrophy, prevention of bone loss, and buildup of LDL cholesterol.

NOT: brain fog, fatigue, low libido, or migraines.

If you are unwell and you have not checked off each of these boxes, no amount of food intolerance testing, mold testing, hormone testing, peptides, herbs, medications, PRP, injections, etc. are going to work.

I can't tell you how many times my patients have started eating right for their genetics, sleeping well, and managing stress and their symptoms clear up or their weight loss begins.

NOTE: I'm talking about general feelings of unwell (overweight, fatigue, acne, skin issues, hormonal issues, etc.) - for life threatening or more serious things, we need the right level of intervention.
