This one really landed differently for me. I do my best to care for each and every one of my patients with the same care I expect for my own loved ones...but our patients with unique needs are deserving of an extra special touch.

I was honored and humbled that this patient's mother trusted me with her daughter's care. Her mother had done an amazing job of protecting, nurturing, and fulfilling the needs of her daughter (now an adult) for years. When she decided to allow me to perform a complex surgery on her daughter to treat an unexpected new voice issue, I did not take that privilege for granted.

Vocal cord paralysis is one of the lesser known, but more common causes of hoarseness. When the nerves traveling to the vocal cords are not functioning properly, the affected vocal cord loses the ability to open, close, or tense up appropriately, and this can have negative impacts on the voice, and in some cases on swallowing and even breathing.

The most common symptom of vocal cord paralysis is a breathy, soft, or rough voice with inability to speak loudly, and running out of breath when talking. In rare cases, both vocal cords are paralyzed (known as bilateral vocal cord paralysis)--this is a very serious condition which causes difficulty breathing, since neither vocal cord can open sufficiently.

There are three main causes of vocal cord paralysis:
• Nerve injury during surgery around the neck, lungs, heart, or esophagus

• Direct compression of a nerve in the neck or chest due to a growth such as a tumor

• Spontaneously on its own without any known cause (we think this is due to a virus that infects the nerve and causes it to swell)

Common treatments for vocal cord paralysis include vocal cord filler injections to temporarily bulk up and reposition the vocal cords, permanent vocal cord repositioning procedures (such as type 1 thyroplasty/medialization laryngoplasty and arytenoid adduction), and reinnervation procedures where a functioning nerve is attached to the injured nerve to allow for nerve regrowth to occur. Voice therapy is often a useful part of treatment as well.

Does your throat just not feel right after getting over a bad cough or cold? Maybe you feel a constant phlegmy irritation or need to clear your throat, or your voice sounds a little off? It's possible that you may have a vocal cord granuloma.

Vocal cord granulomas are non-cancerous inflamed growths that can develop on the vocal cords following repetitive strain, such as a bad cough when you're sick, or chronic voice overuse or excessive throat clearing. They usually develop on an especially sensitive part of the vocal cord, called the vocal process, where the lining of the vocal cord can break down and form an ulcer. The ulcer tries to heal, but because of the repetitive injury caused by continuous talking, coughing, or throat clearing, a ball of inflamed tissue gradually develops there--a granuloma.

Granulomas are best found by seeing a who can perform high resolution of the vocal cords. Once identified, they are usually treated by eliminating the vocally traumatic behaviors, voice rest, and treating associated conditions like acid reflux. In the majority of cases, granulomas will gradually resolve over the course of weeks to sometimes months. In severe cases, the granulomas may get so big that they can start blocking the airway, and in those situations, surgery is required to remove them.

Curious about what to expect during your first visit with Dr. Rafii? Here's everything you need to know to make the most of your appointment:

Before your visit, make sure to fill out the medical intake form in the welcome packet that was emailed to you after scheduling your appointment. Bring copies of any related testing or consultation reports, as well as a list of your current medications and supplements.

On the day of your visit, no special diet or fasting is necessary. Take your medications as usual and allow at least 2 hours for your visit. If it's your first visit, arrive 15 minutes early to fill out the intake forms, or 30 minutes early if you haven't yet done so.

During your visit, you'll have a comprehensive voice evaluation, including a voice history, medical history, perceptual voice evaluation, and a videostroboscopy exam. This state-of-the-art diagnostic exam provides valuable information on the structure and function of your vocal cords and can help identify any issues that may be causing hoarseness or other vocal problems.

After your evaluation, Dr. Rafii will work with you to create a customized voice treatment plan that meets your individual needs. He may also coordinate with other members of your voice team, such as a voice therapist or speech pathologist, if necessary.
