Mun Health

Mun Health Mun Health is building digital tools to address the mental health side of chronic disease care — starting with diabetes distress.

We help people recognize, address, and reduce distress. We make care empathetic, more supportive, and more effective.

Instructions aren’t the same as guidance.

Most care plans are instructions.

Patients need guidance.

That’s what our pilot is focused on.

It’s not that people don’t care.

It’s that care plans are complex.

Complexity without structure leads to avoidance.

Care plans often include:

• Medication changes
• Diet adjustments
• Monitoring
• Exercise
• Follow-ups

That’s a lot to carry without a system.

We’re piloting structured support to help organize and guide care plans between visits.

One appointment.

90 empty days after.

That’s where most care plans quietly fall apart.

We’re building support for that space.

Doctor visits are 30–60 minutes.

Diabetes management is 24/7.

That math doesn’t work without support.

What would make follow-through easier for you?

The appointment is structured.

The weeks after are not.

That unstructured space is where adherence drops.

It’s also where most chronic care actually happens.

We’re focusing on that void.

“I should” is not a plan.

It’s a thought.

Care plans fail when they live in our heads instead of in a system.

Between visits, most patients are managing alone.

We’re piloting something designed to turn “I should” into structured follow-through.

If following a care plan were easy, adherence rates wouldn’t hover around 50%.

So what actually gets in the way?

• Complexity
• Emotional resistance
• Forgetting
• No structure
• No feedback loop

Most people intend to follow their care plan.

They agree with it.
They understand it.
They want the outcome.

But intention is not ex*****on.

There’s a gap between:
“I know what I should do.”
and
“I’m consistently doing it.”

That gap is where health outcomes are won or lost.

We’re launching a pilot focused on closing that gap.

I had knee surgery.

My physical therapist gave me exercises to do at home.

They sat on my desk.

Every day I told myself I’d do them.

I didn’t.

Not because they weren’t important.
Not because I didn’t trust my PT.

But because they were uncomfortable.
Because life happened.
Because there was no structure around them.

This is exactly what happens with diabetes care plans.

We don’t need more instructions.
We need better between-visit support.

We’re building that.

Be honest:

Have you ever left a doctor’s appointment fully intending to follow the plan…

…and then didn’t?

What got in the way?

No judgment. Just conversation.
