Yoga in a creek is a multimodal rehab cheat code. And yes, I know it looks unhinged. Let me explain…

I’m Luke Brohm, a Doctor of Physical Therapy, and I’m rehabbing my own Achilles tendon repair.

So why yoga? Why a creek? Why not just… a clinic or my home gym?

First off: time management I like to spend as much time with my kids as I can, and I was already taking my son to the creek.

Now the the meat and potatoes:

🧠 The rocky creek bed = sensory gold.
Your foot and ankle have mechanoreceptors (tiny sensory receptors that send information to your brain about where your body is in space). After surgery, that sensory loop gets disrupted. The uneven, unstable surface of a rocky creek bottom forces those receptors to wake back up and start communicating again. Research on ankle proprioception consistently shows that unstable surface training improves neuromuscular control and reduces re-injury risk.

šŸ’§ The water = extra feedback to the whole foot. Plus it was cold enough to shock my nervous system and make me want to pull my foot out initially. Fighting through the initial discomfort knowing no damage was going to happen to my foot helps to calm down the brain pain response to prevent complex regional pain syndrome (think of an extreme pain response in absence of damage)

🧘 The yoga = balance and neuromuscular control layered on top. Single-leg balance work is one of the most evidence-backed interventions for ankle and Achilles rehab. Yoga postures that challenge your base of support force your ankle stabilizers, intrinsic foot muscles, and core to all coordinate together.

🐦 The birds? That’s not just vibes.
Natural soundscapes (running water, birdsong) have measurable effects on the autonomic nervous system. Exposure to natural sounds shifts the body toward parasympathetic dominance: lower cortisol, reduced heart rate, and improved recovery state.
Chronic pain and post-surgical rehab are heavily influenced by your nervous system’s threat response. If your brain perceives your environment as safe, healing accelerates. The creek isn’t just pretty. It’s therapeutic.

Understanding how our body functions can help you get to the root cause of almost any pain issue.

Our body wants to survive and reproduce. When you go down the basics, those are the 2 main goals of every body system. But our bodies are designed to live in the wild and we don’t anymore. Understanding this allows you to gain a better grasp why we develop pains, especially chronic pains, and how to better deal with them.

Find a good PT and break your cycle of pain and weakness!

I do love the show, but this scene is a good metaphor for people who have been to multiple medical providers for their pain and not actually gotten into the root cause. I bet the provider they saw gave it their best effort, but sometimes it’s not in the right direction.

Pain and weakness can become cyclical, and getting to the root cause is how you actually break the wheel and set yourself for you from pain.

And I promise I won’t go mad like Daenerys and I’ll write a better ending to the show!

I’m 7 weeks post operative today. I’m also trying to get back to my full life and show up and in my rehab!

2 major safety features in my set up to prevent excess load on my Achilles repair:
-heel wedge/brace on my foot to limit motion at my ankle
-box to prevent low depth to limit load on ankle

By performing a squat in this way, I am able to keep most of the load off my Achilles and through my quads and glutes.

This is 245 pounds but I have been progressing up in weight over previous weeks. I also could squat heavy prior to my injury.

If you are recovering from an Achilles repair, I would be hesitant to squats too early unless you have the proper set up and have cleared it with your surgeon and/or PT.

Most people think you need heavy weight to build strength.
You don’t.

Blood flow restriction training lets you get significant muscle and strength gains using much lower weight. For anyone rehabbing an injury, that’s huge.

Here’s why it works: by partially restricting venous blood flow out of the muscle, you create a metabolic stress environment that mimics what heavy loading does. Our muscles get the signal to grow and strengthen without the tissue stress that could set your recovery back or injury you.

I’m currently using it as part of my own Achilles tendon repair rehab.

The bonus nobody talks about? The workouts are short/quick. Which as a parent trying to squeeze training into whatever 20-minute window the universe gives me, that matters.

Blood flow restriction is a great way to gain strength without actually loading the muscles or tendons without an external load. Very beneficial in early rehab to improve strength and decrease risk of injury.

Also. This girl was getting her rehab in too!
