*COVID 19 Vaccine Clinic*
Puckett Family Clinic will be having a walk in COVID 19 vaccine clinic on Wednesday, August 25 from 8am to 5pm at the Hall Voyer Foundation Building Lyday Hall for anyone who needs a FIRST, SECOND, or BOOSTER dose of Pfizer or Moderna vaccine. The CDC is recommending boosters for individuals with moderate to severe immunocompromised conditions and those undergoing cancer treatments at this time. They suggest waiting 8 months after your second vaccine for healthy individuals. Our website will be updated by Tuesday August 24th. Please go to pfccovidvaccine.com at that time to print off and complete the information forms to bring with you along with insurance information and your white vaccine card if possible. We will also have the updated forms to fill out on site. Thank you and hope to see you Wednesday.
