Job Description
Qualifications:
Graduate of an approved school of practical nursing
Licensed as a licensed graduate Practical Nurse in the state(s) where practicing
One (1) experience as a LPN
Provides means of transportation as required by the nature of the position, possesses a valid State driver’s license and carries auto insurance
Prior Private Agency or Home Care experience preferred
Ability to provide care in a variety of settings
Responsibilities:
-Provides quality nursing care and related services in the home
-Maintains and demonstrates effective client services in an accurate and timely manner
-Adheres to company’s policies and procedures as required
-Performs related accountabilities/responsibilities as required or directed
-Provides those services requiring skills using the Nursing Process (Assess, Plan, Implement, Evaluate)
-Continually evaluates the client’s nursing needs and works with the client/caregiver in developing ways to meet their needs
-Follows a current, individualized plan of treatment by making revisions based on original and ongoing assessments and in conjunction with the doctor, Supervising RN, and client/caregiver
-Informs the physician, Supervising RN, and other healthcare team members of changes in the client’s condition and needs
-Initiates and performs teaching of the client, family, and other health team members, as appropriate, in order to meet identified needs and achieve goals
-Exhibits interpersonal skills which lend themselves to effective communication with other healthcare workers and enhances sensitivity to client needs
-Informs clients of their rights and responsibilities
-Functions under the direction of the Supervising RN and integrates services being provided to each client by all disciplines through participation in team conferences
-Adheres to all company documentation and timelines requirements
-Maintains an accurate account of the medical supplies in his/her possession
-Demonstrates flexibility with assignments
-Knowledgeable of and adheres to company’s policies and procedures
-Understands and complies with safety and equipment management policies, including OSHA regulations and infection control policies
-Maintains awareness of and adheres to nursing policies and procedures
-Participates in in-service programs as required
-Participates as a member of appropriate committee as requested
-Participates in conferences with supervisor as often as necessary
-Participates in Quality Assessment/Improvement activities as indicated
-Performs related accountabilities/responsibilities as required or directed
