I just returned from a week in Dominica, Commonwealth of Dominica — one of the least-visited, most pristine islands in the Caribbean. No cruise ships. No crowds. Just volcanic mountains, black sand beaches, and some of the most extraordinary marine life I have ever seen.

These are my favorite images from beneath the surface.
A Giant Frogfish hiding in plain sight. A Glasseye Squirrelfish staring back at me with that impossible eye. A Christmas tree worm no bigger than a quarter, filtering the ocean one breath at a time. A Spotted Cleaner Shrimp — barely an inch long — tucked into a field of anemone polyps. Creatures most people will never know exist.

Every single one of them stopped me cold.
I am a surgeon by training. Precision, patience, and attention to detail are not just professional habits — they are how I move through the world. Underwater photography taught me that all over again on this trip. You cannot rush the reef. You cannot force the shot. You slow down, you get quiet, and you look. Really look. And when you do, the ocean reveals things that will take your breath away.

This is where the Dominica series ends. And it ends with the smallest creature of all.

That tiny jewel in the centre of the frame — surrounded by thousands of anemone polyps, almost invisible to the naked eye — is a Spotted Cleaner Shrimp (Periclimenes yucatanicus). Less than an inch long. Perfectly camouflaged. Completely extraordinary.
I almost swam right past it.

It took slowing down, getting close, and truly looking to find it. And when I did — it stopped me cold. Because something that small, in a world that vast, managed to be exactly and perfectly itself.
That's what this entire trip reminded me.

The details are everything. In photography, 40 feet below the surface of the Caribbean. And in surgery, where the difference between a good outcome and a remarkable one often comes down to the smallest, most careful choices.

Look at that eye.

This is a Glasseye Squirrelfish — Heteropriacanthus cruentatus — resting on the sandy bottom of a Caribbean reef in Dominica, Commonwealth of Dominica. That oversized, luminous eye is no accident. It evolved specifically to gather light in the darkness of deep water, giving this fish vision that most creatures can only dream of.

It sees everything.

As an oculoplastic surgeon, I spend a lot of time thinking about eyes — what they can do, how they look, and what happens when something gets in the way.

For many of my patients, that something is their own eyelids. Gradually, quietly, the skin begins to droop. The upper lids grow heavy. The field of vision narrows. The face in the mirror starts to look tired — even when you feel anything but.
