Today, I used the just-released Skeletal Dynamics Locking Hand Nail to treat this transverse, fully displaced proximal phalanx fracture in a 59 year-old-man who fell off his dirt bike. The surgery was performed today at the Specialty Surgical Center in conjunction with The Orthopedic Institute of New Jersey. The nail was used for its rotational control of the transverse fracture and to allow for early motion. I also used the Alafair Biosciences , a plant-based hydrogel to limit tendon adhesions. He will start occupational therapy at IVY Rehab. Kenneth Gershberg was the device representative for both implants.

I had an anxious little boy who had a very angulated right small finger fracture. I took him to surgery to reduce and pin the fracture. I also treated his stuffed animal with a splint. Both the patient and the stuffed animal healed quickly.

This 53 RHD F presented with a right distal radius malunion and carpal tunnel syndrome. She fell as a young adult and was casted. The bone healed in a poor position which was giving her painful, reduced range of motion as well as numbness and tingling in her fingers. She was miserable. With the help of Florian Vanquickenborne at , we were able to 3D plan her corrective double osteotomy. I performed carpal tunnel release and a double osteotomy to her distal radius. I utilized the skeletal dynamics geminus distal radius plate. I used bone allograft and the Exogen Ultrasound Bone Healing System. Her bones healed, carpal bone alignment improved, range of motion improved, and her pain was eliminated.

Prior to surgery, I mark the correct side and site to eliminate wrong side/site surgery. Sometimes, with fingers (or metacarpals) there are multiple potential sites on the same hand, so I mark the ones that need surgery, may need surgery, and don't need surgery. This patient had 3 metacarpal fractures, one was obviously rotated and needing fixation, the other two eventually were also found to be rotated in surgery and as such need fixation as well. She did well with the intramedullary screw fixation by Skeletal Dynamics.

WOW!!! I am so excited to present my patient who did phenomenal! She is 27 and fell at work, fracturing her index, middle, and ring finger metacarpals. She was very swollen at presentation. Given her three fractures and severe swelling, I used the intramedullary screw fixation system to fix and immobilize the three metacarpals. This all-internal system allowed her to move her fingers right away at . Almost 3 months later, and the were healed solid. She regained full motion and strength without pain AND she returned to work, full duty. Special thanks to Thadius Lazor who was the rep for the case and my team at the exceptional Specialty Surgical Center who made this possible .

Another successful Distal RadioUlnar Joint Arthoplasty with the Aptis Medical Prosthesis.
This patient had long-standing persistent ulnar-sided wrist pain. MRI showed a tear to her Triangular FibroCartlage Complex (TFCC, the meniscus of the wrist). At her initial surgery, an arthroscopy, we also found terrible arthritis which was not seen on the MRI. Although we repaired the TFCC, her wrist pain slowly returned. After confirming the source of her pain was the DRUJ, with a steroid injection, she agreed to have the DRUJ Arthoplasty. After surgery, she quickly regained painless pronosupination (wrist rotation) "I should have done this years ago!". Another happy patient. This 27 year old patient is active duty in the military and fractured right ring metacarpal. Although the was moderately displaced, he had a notable lag to extension and opted for surgery as he was too swollen for a closed reduction and casting. Given the small canal size of the 4th metacarpal , a standard metacarpal nail was too thick; therefore, I used the largest Micro Nail (normally used for phalanges) to reduce and maintain the fracture alignment. He made a full recovery and quickly returned to active duty.

Osteomyelitis from a dog bite. A 12 year old girl was bitten by the family dog 2 months prior to presentation. The dog’s tooth crushed her nail and punctured her distal phalanx. On presentation, she had florid osteomyelitis ( ) with notable bone destruction. In , we cultured 6 , curetted out the osteomyelitis, and placed Rapid Cure Beads/Paste. She was placed on 6 weeks of IV , and she cured the infection. Final taken 2 months off antibiotics shows resolution of infection and near complete incorporation of the filler from the Stimunlan.

The patient grew out the following bacteria: Fusobacterium nucleatum, bacteroides pyogenes, strep viridans, pasteurella, Eikenella corrodens, staphylococcus aureus

meeting this morning with Florian Vanquickenborne, a clinical engineer at in . We are discussing corrective of my patient's distal radius using the and the plate. I've used this process many times and it works amazingly well. This pre-planning and cutting guides takes away all the guess work. All the cuts and screw holes are preplanned. The plate fits perfectly well. Once finalized by us, the cutting guide plans are emailed from Belgium to Michigan, then they are 3D printed with white Nylon, and eventually mailed to the hospital. They are sterile and ready to use . Amazing technology. The results are wonderful. Florian is great to work with too!
