90-90 is a difficult position to train Hip IR from.

This position called “Supine work smart not hard” achieves the same results.

Protocol:
2 minute stretch

Activate Progressive tissue for 30 seconds or so. That’s the blue arrow so try to externally rotate your leg, but remember it’s an isometric so there should be no movement so push down with the top leg evenly.

Activate regressive tissue for 30 seconds or so. This is the red arrow so you’re trying to actively internally rotate.

Hold a deeper 2 min stretch

Follow up with CARs making sure to really focus on internal rotation during the whole rep.

If by chiropractic care you mean spinal manipulation (not waving chicken bones over the patient or doing aura adjustments) and as part of a multimodal approach that includes education and exercises.

The two videos show Visit 1 spinal motion via Cat-Cow assessment vs Visit 2.

Patient sustained a low back injury event and sought primary care with his MD. Diagnosis initially was a lumbar spine muscle strain, which was inconsistent with our physical exam.

Our physical exam revealed sciatic nerve tension bilaterally, but worse on the involved side, numbness in the lower gluteal region and diminished reflexes on the involved side. This is not something that occurs with a muscle strain.

Treatment consisted of chiropractic adjustments to the lumbar spine (spinal manipulative therapy SMT in the literature) and exercises in-office.

The patient had functional deficits in ADL’s (activities of daily life) such as getting dressed and putting socks and shoes on, and not being able to sit for more than 5 minutes without pain and numbness.

During our second visit, pain dropped from 8/10 to less than 1/10 (whatever your thoughts on the VAS scale), function improved with ADLs and patient could even play with his 5 year old son on the floor. (The five year old loves doing the Cat Cows so double win).

There are stories like this everyday in offices of forward thinking chiropractors who approach clinical care based on evidence and patient centered values.

Before seeking out care, do some research on your own, ask your friends and family for advice if they’re currently seeking care, and try to find a provider that combines passive care interventions like SMT with active care and home based exercises.

You owe it to yourself to find the best and most appropriate care as opposed to the cheapest and closest.
