The Foundations Nashville Intensive Outpatient Program closed its location effective December 8, 2020. For many years, Foundations Nashville provided customized, high-quality care to adults with addiction and/or mental health issues. We thank the physicians and staff for their expertise and dedication to the patients we have been privileged to serve, and look forward to continuing to support the community with needed behavioral health services at Rolling Hills Hospital in Franklin, TN. We continue to offer In-person Intensive Outpatient Services at Rolling Hills Hospital (Phone: 615-807-4059) as well as providing Telehealth services, so that individuals can have their behavioral health needs assessed electronically. Telehealth is a HIPAA-compliant approach that allows you to see and speak with a Nurse or Therapist just as you would during an in-person session— in real time, respectful of social distancing. At Rolling Hills Hospital, we offer morning and/or evening Telehealth programming a few times a week. Patients have the ability to maintain responsibilities while continuing with intensive treatment.
