As a birth doula, my role is to support families through all stages of pregnancy and birth—no matter the delivery plan. But recently, I was reminded of the limitations that still exist in how doulas are viewed and treated in the medical setting.
One of my clients is having a planned cesarean, and despite my expertise in providing emotional, informational, and physical support, I’m treated the same as any other visitor in labor and delivery—meaning I’m not allowed to be with them during the surgery or even wait with them in the OR. This is despite the growing body of evidence showing that continuous support, including before and after a cesarean, is just as important as during vaginal labor.
Research shows that having a doula during a cesarean birth can help reduce anxiety, improve communication, and enhance the overall birth experience. A study published in The Cochrane Database of Systematic Reviews found that continuous support during labor is associated with shorter labors, less use of pain relief, and higher satisfaction with the birth experience. These benefits extend beyond vaginal births—doulas can play a crucial role in providing comfort and emotional reassurance before, during, and after a cesarean birth, especially when hospital protocols prevent other forms of support.
It’s time to advocate for expanding doulas’ roles in all birth settings—because every family deserves continuous, compassionate support, no matter the birth plan. Our skills for emotional support can really help clients especially when anxiety is high before surgery or when a planned cesarean is delayed causing increased tension.
