Young patient with hip dysplasia. Difficult surgical problem, shallow acetabulum with retroversion and posterior bone loss. Increased femoral offset, small canal, and a need for more anteversion. Large leg length discrepancy. Advancements with robotics and modern stem designs allow for accurate cup placement with minimized uncoverage and measured anteversion of a proximal loaded femoral stem to confer stability… to be continued on the left hip!

Bad things happen to good people. 41 year old who is 7 weeks out from a hip fusion takedown and THA that sustained a Vancouver C periprosthetic femur fracture after a fall. Unfortunately, a case that requires a long plates, bunch of screws and cables to fix. Hopefully, on to a path to a clean recovery from here

Another good example of robotic technology at work. A patient with a long standing hip deformity resulting in severe arthritis and superior migration of the hip. What would be very difficult to manually recreate the hip center and achieve appropriate cup placement due to obscured anatomic landmarks is made significantly easier with the Mako robot.

Femoral neck (hip) fractures unfortunately are very common. But still some controversy remains with regard to screw fixation vs Arthroplasty. Many factors help surgeons decide include type of fracture, age, life expectancy, activity level, medical issues. For this case, a younger 65 year old male, sustained a fall off a bike. A higher demand patient with limited health issues. His fracture, impacted with and displaced on CT would have increased risk of failure with screws. So for him, an acute total hip replacement was performed. He is able to walk next day, has minimal pain, back to biking within upcoming weeks!

Another great example of robotic technology at work. Patient with prior surgery and development of arthritis/AVN. Previous plate and screws were able to be retained and a total knee replacement preformed. Traditional methods would have required another surgery to remove hardware prior to replacement or placement of a revision type femoral implant. But with emerging technologies, single staged, one surgery, primary total knee implants can be achieved!

Total hip replacements are life changing! A case of a patient with arthritis of her right hip which was replaced 3 weeks ago. She had a muscle-sparring, superior approach to the hip. She took “one pain pill post operatively, is back to 90 percent of her activities, has not used a walking device for weeks, is driving and feels great.”

A wonderful example of how the combination of a minimally invasive hip approach and a motivated patient can produce excellent early outcomes!

A gold standard prospective-randomized study in JBJS looking at midterm follow up for cementless total knee arthroplasty. New biological designs allow for your own bone to grow into the prosthesis, theoretically leading to increased longevity of your knee replacement! There is also promise of a shorter surgery and decreased complications compared to traditional cementation. Good work coming out of Wash U!

The power of the MAKO robot at work today. A difficult case of a proximal tibial malunion with fixed flexion and valgus deformities. Through robotic preplanning, we were able to retain the intramedullary nail for a TKA. Intraoperatively, we were able to precisely balance the gaps and place the tibial component without need for a traditional two-staged procedure. Now on the road to recovery!

Louisiana State Medical Board has halted operations for elective total joint cases for at least the next 30 days. Certainly, hip replacements are designed for all shapes and sizes but a challenging patient population are those with short stature. A paper I was involved and recently accepted to Journal of Arthroplasty, demonstrated that while operative times may be longer for surgeons, successful long term outcomes can be achieved for these individuals!
