That’s what he told me at his 12-month follow-up.
He is in his late 20s. Early hair loss with the temples receding, midscalp thinning. The kind of pattern where you’re still young enough that it hits differently.

We performed an FUE procedure. 2,535 grafts.

This patient underwent a lower face and necklift with one goal in mind: restore her youthful appearance without changing who she is. Refreshed, natural, and authentically her.

But here’s what makes this case particularly educational: During surgery, we identified an enlarged asymmetric parotid gland (a normal anatomic variant that some people have).

This is a perfect example of why not everything in the face can or 𝘴𝘩𝘰𝘶𝘭𝘥 be altered completely.

𝗧𝗵𝗲 𝗣𝗮𝗿𝗼𝘁𝗶𝗱 𝗥𝗲𝗮𝗹𝗶𝘁𝘆:
The parotid gland has two lobes (superficial and deep) and produces saliva essential for digestion and oral health. It sits intimately with the facial nerves, which control all facial movement. Attempting to reduce it carries significant risk: facial nerve injury, salivary fistulas, and functional impairment. The risk far outweighs any aesthetic benefit.

𝗢𝘁𝗵𝗲𝗿 𝗦𝘁𝗿𝘂𝗰𝘁𝘂𝗿𝗲𝘀 𝗪𝗲 𝗥𝗲𝘀𝗽𝗲𝗰𝘁:
• Masseter muscle bulk (controls chewing; over reduction affects function)
• Bone structure (zygomatic arch, mandible angles; augmentation has limits)
• Facial nerve branches (dictate what’s surgically feasible)
• Natural fat compartment asymmetries (minor differences exist in everyone)

❓Ever notice how your eyes can make you look more tired than you actually feel?

That was the case here. He is healthy and full of energy, but the under eye bags gave a different impression.

We addressed it with a lower eyelid lift and fat transfer. The result is a clearer and brighter look that better reflects how he feels.
