Patients who are neither hospitalized nor on hospice care need access to palliative care in a community clinic or office setting, such as a cancer center or dialysis unit. Such services may be delivered by physician groups or hospice- or home-health-based palliative care teams, or as extensions of an inpatient palliative care team and program.

Some office/clinic palliative care programs are embedded within another specialty clinic (for example, an oncology clinic), while others may share space within an existing clinic but operate independently; still others are geographically freestanding. Close to 14,000 people in the U.S. are diagnosed with cervical cancer each year—most frequently in women aged thirty-five to forty-four with the average age being close to fifty. Many cases are caught in the early stages, thanks to preventive screenings like Pap smears and HPV testing. These measures have helped cut the cervical cancer mortality rate in half since the 1970s.

Palliative care teams often consult on more advanced cases, which may include patients who have not had regular access to preventative gynecologic care because of financial barriers, lack of insurance, geography, or other disparities. That said, advanced cancer also occurs for patients who did receive preventative care. Regardless of the stage, it’s essential that we are prepared to support all our patients with the full range of tools that we can provide—from symptom and pain management to psychosocial and emotional support.

High-quality palliative care isn’t defined by program size or budget—it’s defined by consistency, collaboration, and a shared commitment to patients and families. Across the country, smaller palliative care programs are demonstrating what’s possible when teams focus on training, teamwork, and outcomes that matter.

In summer 2025, CAPC spoke with twenty smaller organizations—community hospitals, critical access hospitals, independent hospices, and small practice groups serving fewer than 150 beds—to understand how they sustain their commitment to high-quality palliative care. Their experiences highlight what makes these programs effective: clear structures, intentional learning, and a deep sense of accountability to their communities.

We’ve all heard the stereotypes—passed on through jokes or gritted teeth—about the different generations. “Baby boomers are stuck in their ways and reinforce hierarchy.” “Gen Xers are off on their own playing a video game.” “Millennials think they should have it all but don’t want to work for it.” “Gen Z thinks the world exists on a smartphone screen.”

Are these stereotypes helpful? Of course not. And the year we’re born is only one of countless factors that shape who we are. Yet our age and everything that comes with it—world events, cultural trends, and technologies, to name a few—influence how we’re raised and see the world. And we bring all of that to our work.

We chose to write about the topic of intergenerational teams because we think about it and talk about it a lot. The CAPC team, like many health care teams, ranges in age from colleagues in their twenties to those in their seventies. To be effective, we need to leverage that age diversity for its strength while creating an environment that can accommodate the different styles and expectations of the group.

This two-hour, case-based, interactive clinical workshop offers clinicians (physicians, advanced practice providers (APPs), and registered nurses) practical information on using buprenorphine to manage pain in patients living with a serious illness and substance use disorder. The workshop will cover:

🔹 An overview of buprenorphine and how to take advantage of its unique pharmacology for patients with substance use disorder
🔹 The complexities of managing acute pain while on buprenorphine
🔹 Evidence-based dosing, with guidance on opioid rotations to and from buprenorphine
🔹 Practical tips on effective prescriptions and navigating buprenorphine access

The workshop concludes with an open question and answer session for clinicians to ask about all aspects of buprenorphine and share clinical experiences.
