This month, for the first time in six years, colleagues and friends gathered in Philadelphia for the CAPC National Seminar for three days of learning, connection, and collaboration.

In addition to didactic sessions and workshops, 129 teams presented their projects at a poster session on September 16th. Of these, 12 were honored with Special Recognition for their innovative approaches, creative resource utilization, and promising outcomes poised to drive progress in the field. Poster projects and initiatives ranged from localized team-based quality improvement to system- or state-level collaborations.

Through this blog, we aim to spotlight the resourcefulness demonstrated within the palliative care community—and offer a source of inspiration for other programs. The projects described below fall under four domains: Equity/Accessibility, Technology/Data-Driven Care, Quality/Systems Improvements, and Education/Workforce.

The ongoing challenges within the U.S. health care system continue to pose an existential threat to clinician well-being, with mounting pressures leading to increased rates of depression, anxiety, burnout, moral distress, moral injury, trauma, grief, etc.

The Health Care Professional Well-Being toolkit provides resources that clinicians and teams can use to support well-being. CAPC firmly believes that it is the responsibility of organization, state, and federal leaders to create a more supportive and sustainable environment for health care clinicians. We have included individual resources to normalize your experiences and help bridge the gap if you are struggling. However, we urge leaders to consider participating in advocacy and initiatives that can help change the U.S. health care landscape.

The Health Care Experience of Hispanic Patients with Serious Illness: A Literature Review → https://ow.ly/pRHg50UUaP9

CAPC has conducted a first-of-its-kind literature review analyzing more than 200 peer-reviewed journal articles to understand the health care experience of Hispanic patients with serious illness, and their families and unpaid caregivers. that sentence, we need to adjust to: The literature review identified 14 key findings, including areas where care for Hispanic patients often falls short such as pain and symptom management and patient-provider communications.

Here’s how you can leverage the literature review to drive meaningful change:
💙 Health Care Professionals: Gain insight into disparities affecting Hispanic patients and caregivers to develop interventions that address domains such as language barriers, cultural considerations, and communication gaps.
💰 Payers and Policymakers: Target professional training and process improvement efforts to the areas where inequitable experiences are most significant.
📰 Researchers: Identify research gaps in care outcomes (e.g., pain management, communication) and study interactions across patient, provider, system, and societal factors to address disparities.
⭐ Hispanic Community Leaders: Understand systemic disparities for Hispanic patients, and access the data needed to identify where additional support and tools may be needed to combat these disparities in your community.

This literature review is part of CAPC’s Project Equity initiative, and the findings will be used to develop pathways and tools to create meaningful change and reduce inequities experienced by traditionally marginalized communities.
