MANUAL THERAPY

One of the many manual therapy techniques we employ at IPT is the subscapularis (subscap) release, shown above. The subscapularis is a rotator cuff muscle that resides on the inside of the shoulder blade. This muscle rotates the shoulder internally, towards the midline of the body. Sometimes the subscap can become stiff and painful when the shoulder is kept in internal rotation for too long. For example, long periods of sitting or shoulder disuse can cause this. The Subscap often refers pain to the mid back, shoulder blade and shoulder itself. Releasing the subscapularis through therapeutic massage can provide muscular blood flow improvement and decrease pain.
HAMSTRING RELEASE EXPLANATION
At Integrative Physical Therapy of NYC we utilize various manual therapy techniques to improve patient functionality and daily living. For stubborn hamstring muscle tension and trigger points, our therapists can apply pressure to the hamstring and straighten out the knee while the muscle is under this light compression. This provides the patient with relief as tight, "knotted", parts of the hamstring are targeted for stretching and elongation.

Intrinsic hand strengthening
What are intrinsic hand muscles? They are the small muscles that reside on the inside of the hand which control crucial hand movements. Our hands would be essentially useless without them. The intrinsic hand muscles are broken up into 4 parts; the thenar and hypothenar eminences, the interossei and lumbricals. The thenar muscles are kept at the meaty part of the thumb, the hypothenar muscles line the outside of the fist (by the pinky finger), and the interossei and lumbricals flank the deep sides of the hand bones. These four muscle groups work together to perform paramount movements such as separating and consolidating the fingers, and grasping, holding and pinching objects. Injury to the hand in the form of nerve impingement, fractures, strains, tears, etc, can disrupt the balance between these muscles and thus disrupt normal hand function. We use numerous ways to strengthen and restore the balance between these muscles for our patients at Integrative Physical Therapy of NYC. NECK PAIN AND MANUAL THERAPY
At Integrative Physical Therapy of NYC (IPT), we commonly see patients with chronic neck pain and headaches. In a recent study by Bernal-Utrera, et al. evidence was found to support the use of therapeutic exercise and manual therapy techniques to treat these symptoms. Physical therapist guided manual therapy was confirmed to decrease perceived pain faster than therapeutic exercise, and therapeutic exercise was found to reduce disability faster than manual therapy. So what do we do with this conflicting information? Give the patient both!!

Strong middle and lower trapezius musculature is crucial to keeping our shoulders pain free and posture optimal. The prone YTI exercise targets these muscles specifically. If you have rounded shoulders, or spending too much time in a seated position (as 90% of us do) try performing this exercise for 2 sets of 12 reps, every other day!

ERGOCYCLE
At Integrative Physical Therapy of NYC we have the option to use our Ergocycle machine to help our patients reach their goals. The Ergocycle can be operated by the arms or legs, and is utilized as a simple warm up for a patient's exercise routine, to increase general blood flow, and to improve stubborn/stuck range of motion at the elbow, shoulders, and knees!
MECHANICAL THERAPY
At Integrative Physical Therapy of NYC we often see patients with neck pain combined with traveling numbness and tingling symptoms seen in the arms and hands. These symptoms indicate a cervical nerve root impingement and are commonly called cervical radicular symptoms. If warranted, we can use mechanical cervical traction to help decrease nerve root impingement and provide our patients relief. It is important to note that the mechanical traction does not help the patient alone.
RESISTANCE BANDS SIDE STEPS
Side-stepping with a resistance band is an excellent exercise used at Integrative Physical Therapy of NYC to activate the gluts. Simply place a looped resistance band around the ankles and side step 20-30 feet, always keeping the band taut. Activation of the glut medius and maximus helps strengthen them as well as decrease knee and hip pain. For an extra challenge, try looping the resistance band around the feet!

Stand one inch away from a wall, raise your hands above your head and try to squat down until your thighs are parallel with the floor.
