In this new series, “Why I Might Tell You ‘No,’” I’ll cover a broad range of procedures and explain why I might decline to perform them. This week we are talking about: the “Mounjaro Makeover.” A “Mounjaro Makeover” addresses skin laxity caused by weight loss. I might say "no" if you haven't reached your weight loss goal yet because if you continue to lose weight, for example if you lose an additional 30 pounds after your procedure, you may be left with the same laxity which we just worked so hard to restore. If you’re considering a breast augmentation, but prefer to avoid implants, take a look at this story featuring my patient, Heather’s, fat transfer breast augmentation experience. After significant weight loss through diet, exercise, and GLP-1 use, Heather found that her breasts had “deflated.” During her consultation, she shared that she wasn’t interested in traditional implants as she was reluctant to introduce foreign material into her body. The only way to spot-reduce fat is with liposuction. Thanks to and for including me in their “awake” plastic surgery story. Sometimes “awake” plastic surgery is a great idea. Sometimes... less so. For short procedures like fat transfer breast augmentations and small areas of liposuction, trading general or “twilight” anesthesia for local can be easier in many ways. For larger procedures, anesthesia is recommended. It’s important to seek a board-certified plastic surgeon to guide you on this and your other surgical decisions.

We perform a Brazilian Butt Lift using AlloClay, an off-the-shelf fat graft alternative, on a fit patient seeking an athletic enhancement. We demonstrate the importance of ultrasound guidance to safely navigate the different tissue layers—from skin and subcutaneous fat to the gluteus muscle—while avoiding large blood vessels that could cause fat embolism. By placing the graft in the deep fat layer, we achieve natural projection and curvature without the flat, pancake-like appearance that can result from superficial placement. What is the difference between a surgical and non-surgical “mommy makeover”? Surgical mommy makeovers provide superior and longer-lasting results compared to non-surgical options, making the surgical option the ultimate choice for women seeking dramatic, comprehensive post-pregnancy body transformations.
