Ketam*ne is a promising tool in psychedel*c-assisted therapy, and it's also a substance that can cause dependence and harm. Both things are true, and the field can't afford to look away from either.

At last week's InnerMost Mixer in Gramercy, Andrew Tatarsky, PhD and Casey Paleos, MD sit with that paradox. Integrative Harm Reduction Psychotherapy doesn't pretend this tension doesn't exist, it's about holding it with honesty and highly personalized care.

The evidence is there: psychedel*cs, used in the right set and setting, with the right therapeutic container can help people address the root of what's driving their suffering—even harmful relationships with substances. This work requires a clinical lens that centers the whole person.

During last week's InnerMost Mixer, we had the pleasure of sitting down with Andrew Tatarsky, PhD—founder of Integrative Harm Reduction Psychotherapy—for an intimate fireside chat and Q&A with ~40 clinicians who attended.

Casey and Andrew explored some of the most pressing questions at the intersection of harm reduction and psychedelic-assisted therapy: what the disease model gets wrong about addiction, why people develop drug solutions rather than drug problems, the deep philosophical alignment between IHRP and psychedelic work, and what the field still needs to figure out about treating people who have both a psychedelic therapy indication and a complex relationship with substances.

On April 18th, President Trump signed an executive order to dramatically accelerate access to new medical research and treatments based on psychedel*c therapies, directing the FDA to fast-track review of compounds like ps*locybin and iboga*ne.

The FDA will issue national priority vouchers for three psychedel*cs, potentially cutting review timelines from months to weeks. This is the first time it has ever fast-tracked any psychedel*c substance. The science has been building for years, and now the regulatory environment is beginning to catch up.

We've been preparing for this moment at InnerMost. Ketam*ne-assisted therapy is already here, but we've always known it was just the beginning. That's why treatment is one of our core pillars, and training and clinical research exist alongside it. The arrival of new psychedel*c therapies won't just require access, it will require providers who are prepared, protocols that are evidence-based, and a field that can scale responsibly.

Last week’s InnerMost Mixer with Julie Holland, MD was a special evening for attendees and our team.

From a fireside conversation on women’s mental health, connection, and psychedel*cs, to an engaged Q&A, the room was filled with clinicians showing up with curiosity and a shared commitment to doing this work well.

There’s a palpable energy that emerges when you bring together providers across disciplines. It’s less about having all the answers and more about asking better questions, learning from one another, and building an ethical future for offering psychedel*c-assisted therapy.
