Is Your Hair Follicle Going Through Menopause? 💇‍♀️
Might there be a link between menopause and changes in the hair follicle (HF)?

Menopause, which usually occurs between the age of 45 and 55 years, is associated with falling estrogen levels due to ovarian follicle depletion. During this time, hormonal fluctuations and aging can impact the HF, leading to thinning, loss of volume, and changes in hair texture. However, not all women experience these changes, and the extent to which they occur varies considerably from person to person, modified by genetic factors, stress, diet, and other components.

Menopause mirrors the aging process, affecting metabolism and blood flow to the HFs, influencing the availability of vital nutrients. Menopause and the change in hormone levels impact the hair growth cycle and common hair conditions. However, further research is required to understand the therapeutic targets and the role of hormone therapy.

Dihydrotestosterone is a vital hormone in the sexual development of people assigned to males at birth (AMAB). More specifically, DHT is an androgen — a hormone that stimulates the growth of male characteristics.

Your body converts about 10% of your testosterone (the primary androgen) into DHT daily as an adult. This occurs in the ge***al skin and prostate in people AMAB and the skin in people AFAB (assigned female at birth). It also happens in other parts of your body, such as your liver.

Levels of DHT are naturally much higher in people with AMAB than in people with AFAB because they naturally have more testosterone.
What happens when DHT levels are high?
Certain health conditions are associated with high levels of DHT, including:
 
Benign prostatic hyperplasia
Prostate cancer
Androgenic alopecia (male pattern hair loss)
Polycystic ovarian syndrome (PCOS)
 
DHT and androgenic alopecia
Androgenic alopecia is commonly known as male pattern hair loss. The hair loss usually happens on the top and frontal regions of your scalp, causing your hairline to recede over time.
 
Increased DHT activity in your hair follicles is partly responsible for this hair loss and other factors, including genetic ones. High levels of DHT can shrink your hair follicles and shorten the hair growth cycle, resulting in hair loss.
 
Dihydrotestosterone (DHT) is essential to sexual development for people assigned male at birth. But if levels are too high or low, they can cause specific issues like underdeveloped external ge***alia, prostate enlargement, and hair loss. Many of the commonly prescribed prescription medications can cause temporary hair loss, trigger the onset of male or female pattern baldness, and even cause permanent hair loss.  In most cases, alopecia from medications is usually reversible, with nonscarring diffuse hair loss within days or weeks of starting a new drug or a change in the dose. A person who experiences drug-induced alopecia will usually notice thinning hair instead of patchy hair loss more apparent at the crown.
Different drugs cause hair loss at different stages of the hair growth cycle. The majority of meds that impact hair loss affect hairs in the resting (telogen phase), while others affect hairs in the growth (anagen) phase.
Drugs that have been associated with resting phase hair loss include:
·Amphetamines
·ACE Inhibitors
·Antidepressants (particularly SSRIs)
·Antifungal medications
·Antiseizure
·Beta Blockers for heart and glaucoma
·Birth Control Pills
·Blood Thinners (heparin and warfarin)
·Cholesterol lower agents
·Thyroid
·Pepcid and other stomach medications
·Hormone Replacement Therapy
·Accutane and other vitamin A-based medications
·Parkinson’s disease medications
·Nonsteroidal anti-inflammatory drugs (NSAIDs)
·Steroids
 
The only way to know that medication is the cause of hair loss is to stop the medication and realize that it can take six months for the hair to stop shedding. You should start to see new hair growth within three to six months. Your provider should be brought into the loop before you consider this measure. In most cases, lowering the dose or a different medication may treat the underlying condition without the impact on your hair.  Talk to your healthcare provider before starting a new treatment and ask if they are known to contribute to hair thinning or loss. In addition, seek out reference books for pills and medication guidebooks for listed side effects.

September is Alopecia Awareness Month‼️🪮

Alopecia is a term we use in medicine for losing your hair or going bald. There are many types from male patterned baldness or female patterned baldness to balding caused by chemotherapy or a medication. Of the many types, male patterned baldness is the most common and can start in your late teens and progress over the years. Unfortunately, the earlier you start shedding hair, the balder you inevitably become.

There are many other causes for hair loss as well, such as:

-physical and psychological stress brought on by major illness
-surgery
-experiencing a major life event and it will come on quickly
-chronic diseases such as diabetes
-autoimmune disorder.

Some hair loss is permanent while others will reverse themselves or correct with proper treatment. These treatments include oral medications, low-level laser, Platelet-Rich Plasma (PRP), and by Follicular Unit Extraction such as Neograft.

Did you know that skin cancer is the most common type of cancer? Did you know that it can often be found in the scalp?

There are three major types of skin cancer — basal cell carcinoma, squamous cell carcinoma and melanoma. Melanoma typically appears in the skin’s outermost layer (the epidermis) first, then invades the layer below the epidermis, called the dermis.

Melanocyte stem cells, the source of melanocytes, are found in the lower portion of hair follicles. Recent studies suggest that melanoma can arise from melanocyte stem cells in hair follicles as well as from melanocytes in skin layers.

“By confirming that oncogenic pigment cells in hair follicles are a bona fide source of melanoma, we have a better understanding of this cancer’s biology and new ideas about how to counter it.” -Dr. Mayumi Ito at NYU Langone Health

PCOS is the most common endocrine disorder for women of reproductive age. It can manifest itself
clinically as acne and excess hair on the face and body, though many with PCOS experience hair thinning
and hair loss on the scalp. We refer to this as female pattern baldness or androgenic alopecia. This hair
loss is related to a genetic predisposition and hair follicle sensitivity to androgens such as testosterone.
This sensitivity will decrease the growth of hair on the scalp and with PCOS, increase the growth of hair
on the face and body when androgen levels are high. These issues are obviously concerning to most
women and should be addressed by practitioners well-versed on the issue.
