New Research Alert:

Torso-Weighting Effects on Ankle Muscle Activation in Individuals with and without Multiple Sclerosis: Preliminary Findings

presented in APTA CSM 2025 Houston Texas.

Results:
All participants were female with an age range 61 to 71 and Expanded Disability Status Scale range 4.5 to 6.5 for the two individuals with relapsing-remitting MS.

All participants reported an increase in the perceived LOS (0=falling, 10=stable) wearing the BBTW, from rate of perceived stability range of 5 to 7 pretest during eyes closed with no BBTW to a range of 8 to 9 posttest. An
upward trend in the mean and maximum muscle activity was observed in the EMG measurements of both TA and GM in the two individuals with MS and two healthy controls during the Romberg test with BBTW. There was a reduction in center of pressure (COP) displacement in the x and y axes in all participants.

Purpose: To assess mobility and balance.

Equipment Needed: A stopwatch, a standard chair, and a distance of about 3 meters (10 feet) marked on the floor.

Instructions:

Starting Position: Sit in the chair with your back against the backrest.

Begin the Test: When ready, say โ€œGoโ€ and start the stopwatch. Stand up from the chair.

Walk: Walk at a comfortable pace to the marked line (3 meters away).

Turn Around: Turn around at the line and walk back to the chair.

Sit Down: Once you reach the chair, sit down again.

Stop the Timer: Stop the stopwatch as soon as you sit down.

Scoring: Record the time taken to complete the test. A time of 12 seconds or more indicates a higher risk of falling.

Begin the Test: When ready, say โ€œGoโ€ and start the stopwatch.

Stand Up: Stand up fully and sit back down. Repeat this as many times as possible within 30 seconds.

Counting Reps: Count each complete stand (from standing up to sitting down) as one repetition.

Scoring: Record the total number of times you stood up in 30 seconds. Fewer than 8 repetitions may indicate a higher risk of falling.

Purpose: To evaluate balance.

Instructions:

Position 1: Stand next to a wall or sturdy chair for support. Stand with your feet together (heels touching).

Hold for 10 seconds.

Position 2: Stand with your feet in a semi-tandem position (one foot directly in front of the other, heel touching toe).

Hold for 10 seconds.

Position 3: Stand in a full tandem position (one foot directly in front of the other).

Hold for 10 seconds.

Position 4: Stand on one leg (with the opposite foot raised off the ground).

Hold for 10 seconds.

Scoring: If you can hold each position for the full 10 seconds without losing balance, your risk is lower. Difficulty holding any position may indicate a higher risk of falling.

Stand Up: Stand up fully and then sit back down. Repeat this process five times as quickly as possible.

Stop the Timer: Stop the stopwatch when you sit down for the fifth time.

Scoring: Record the time taken to complete the five repetitions. A time of 12 seconds or more indicates a higher risk of falling.

Begin the Test: Start the stopwatch as soon as you lift your foot.

Hold the Position: Try to hold the position for as long as possible without touching the raised foot to the ground or using support.

Stop the Timer: Stop the stopwatch when you either lose balance, touch the raised foot to the ground, or reach 30 seconds.

Scoring: Record the time you were able to maintain balance. Holding for less than 10 seconds may indicate a higher risk of falling.

Happy Balance Awareness Week!

Balance involves the coordination of several systems in the body.

Here are FIVE key systems that help balance:

1. Vestibular System: Located in the inner ear, this system detects head movements and helps maintain equilibrium by sending signals to the brain about motion, head position, and spatial orientation[1]. This system is usually tested in narrow base support activities such as single limb stance, walking heel toe, and unstable surfaces especially with eyes closed.

2. Visual System: The eyes provide critical information about the environment, helping to identify obstacles, judge distances, and perceive motion. This visual input is essential for maintaining balance[1]. This system also provides verticality. Here is a simple exercise to learn about verticality. Squint your eyes together: Notice the horizontal and vertical images seen. The vertical and horizontal image provide information of upright control.

3. Proprioceptive System: This system involves sensors in your muscles and joints that send information to your brain about the position and movement of your body parts. It helps you understand where your body is in space at any moment[2] that including without vision.

4. Central Nervous System: The brain and spinal cord process information from the vestibular, visual, and proprioceptive systems to coordinate movements and maintain balance[3]. Imbalance can be caused when the information transmitted to the brain is not accurate due to a problem with one or more of the balance systems mentioned above.

5. Musculoskeletal System: Strong muscles, bones, and joints are crucial for maintaining balance. They provide the necessary support and stability to perform movements and recover from disturbances[3]. Core stability, and a strong lower body help with balance control.
